Therapist

• The Clinician applies psychological/counseling principles in assessment, clinical interventions, research, training, case management, and program development.
• Responsibilities will include diagnosis, evaluation, treatment, and discharge planning.
• The clinician provides a variety of interventions, such as individual, group, child, and family therapy.
• Interventions will focus on short-term problem-solving therapy, short-term cognitive behavioral therapy, brief psychodynamic therapy, crisis intervention, coping skills, and substance abuse treatment.
• The clinician collaborates with the patient to define treatment goals and measures of success by utilizing evidence-based outcome measures.
• The clinician attends various conferences/workshops concerning mental health treatment, and may be involved in several research studies or multidisciplinary projects.

General

• Providing support to the Medical Management department to ensure the timeliness of outpatient or inpatient referral/authorization processing per state and federal guidelines
• Performing troubleshooting when problem situations arise

Telehealth Nurse

• The LVN provides direct patient care under the direction of a Registered Nurse or Physician.
• The LVN tasks include vital signs, phlebotomy, gathering data, and medication administration.
• The LVN verifies medication for Medical Assistants within the LVN's scope of practice.

Health Coordinator

• Provide perinatal and newborn education and counseling services through the CPSP.
• Develop and structure seamless interface among various services to provide easy access to patients.
• Represent the clinic in all matters dealing with these services.
• Work closely with Clinic Director, Women’s Health Director, Medical Providers, and Nursing staff to develop and implement goals and objectives of the programs.

• Assigned codes to patient symptoms, diagnosis, operations, and treatments to process reimbursements
• Knowledge and expertise in reviewing and adjudicating coding services, procedures, and diagnoses on medical claims
• Completes accuracy and timely entry of ICD-9-CM, HCPCS procedure codes, and CPT codes into the NextGen system.

Community Health Worker

• The Community Health Workers (CHW) are trusted members of their community who help address chronic conditions, preventive health care needs, and health-related social needs.
• Help patients address the social, medical, and community problems that lead to poor health.
• Support patients with health care navigation, specialty appointments/referrals, PCP appointments/establishing routine care and preventative health screenings.
• Provide culturally appropriate health information on topics related to chronic disease management and prevention.
• Receive training in Epic & Essette, DHCS requirements, trauma-informed care, motivational interviewing, and more.

• The Project Manager is primarily responsible for the effective planning, managing, and implementation of highly visible projects to improve enterprise infrastructure and promote service integration of AltaMed’s clinic services.
• The Project Manager is assigned projects that are broad in scope and involve multiple departments across regions.

• Provides support for all operations and initiatives critical to enterprise-wide Compliance Program needs.
• Supports compliance functions, including preparation for, and coordination of regulatory and other external audits.
• Investigates and responds to allegations of non-compliance.
• Delivery of compliance training, policy and procedure drafting, maintenance, auditing, and monitoring.
