About the role

  • Lead Care Manager serving as primary point of contact, coordinating with doctors, specialists, pharmacists, social services, and caregivers to align client needs and care.
  • Manage client cases, coordinate health care benefits, provide education, and facilitate timely, cost-effective access to care.
  • Collaborate with caregivers, providers, and Care Team to promote wellness, recovery, independence, resilience, and member empowerment.
  • Hybrid role requiring travel throughout the San Francisco Bay Area including San Francisco, Marin, and San Mateo Counties.
  • Assess member needs across physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community referrals.
  • Oversee development of client care plans and goal setting.
  • Offer services in-office, via telehealth, or field-based where members reside or seek care.
  • Connect clients to social services and supports; advocate with healthcare professionals.
  • Utilize evidence-based practices (Motivational Interviewing, Harm Reduction, Trauma-Informed Care).
  • Conduct outreach and engagement; log activity in CRM.
  • Evaluate progress, update SMART goals, provide mental health promotion, arrange transportation.
  • Complete documentation and maintain EMR and other business systems; complete monthly reporting; attend training.

Requirements

  • Willing and able to work Monday-Friday 8:30am-5:00pm, both in the field and remotely.
  • 2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations
  • Working knowledge of government and community resources related to social determinants of health
  • Clean driving record, valid driver's license, and reliable transportation
  • Excellent oral and written communication skills
  • Positive interpersonal skills required
  • General computer skills and working knowledge of Google Workspace, MS Office and the internet
  • Bilingual (English/Spanish) preferred
  • Background check and possible drug/health screenings required
  • COVID-19 and influenza vaccination required for patient-facing roles (accommodations may be considered)
