About the role
- Conduct comprehensive assessments of patients’ FND, behavioral health, psychosocial needs, and risk factors
- Develop individualized care plans in collaboration with patients, FND Program multidisciplinary team and outside care providers
- Assist with formulating patient-centered goals addressing FND recovery, mental health needs, social determinants of health, and barriers to care
- Coordinate care with medical providers, mental health professionals, social workers, and other healthcare teams
- Facilitate referrals to community resources, support services, and behavioral health programs
- Monitor patient progress, follow up with patients, and adjust care plans (includes contacting patients, filling out FMLA paperwork)
- Educate patients and families about treatment options, coping strategies, and resources; advocate for patient needs
- Ensure compliance with organizational policies, HIPAA, and relevant regulations; participate in quality and process improvement initiatives
- Inform FND Program Director and Business Manager of behavioral health service line budgetary and resource needs; develop sustainable resource management database
- Work with Clinic Coordinator to develop and maintain statewide FND Resource Collaborative
- Collaborate on community educational engagement activities, webinars, and development of educational materials
- Support clinical research, quality improvement, scholarly work, and supervision of trainees as part of academic mission
Requirements
- Principal Professional: Bachelor's degree from an accredited institution and 3-5 years of experience in clinic case management or highly related work
- Senior Professional: Bachelor's degree from an accredited institution and 2 years of experience in clinic case management or highly related work
- Professional: Bachelor's degree from an accredited institution and 1 year of experience in clinic case management or highly related work
- Prior experience working with families and caregivers (required at all levels)
- Must be able to work cooperatively with all constituencies in the UCHealth Hospital, Department of Psychiatry, and Department of Neurology
- Advanced degree (Masters or Doctorate) may be substituted for experience on a year-for-year basis if directly related to the work assignment
- Skills in risk assessment, including suicide and violence risk assessment (preferred)
- Experience caring for functional neurological disorder (conversion disorder) or other somatic symptoms (preferred)
- Prior experience delivering trauma-informed care (preferred)
- Prior experience working in multidisciplinary teams (preferred)
- Comfortable with public speaking (preferred)
- Prior experience working within academic medical setting (preferred)
- Prior experience delivering virtual treatment (preferred)
- Familiar with federal and state programs and resources (e.g., Medicaid) (preferred)
- Memberships: Functional Neurological Disorders Society and National Society of Social Work (preferred)
- Ability to chart patient visits using an electronic medical record, triage and refer patients, return phone calls/emails, and participate in quality- and performance assessment projects
