About the role
- Conducts comprehensive assessments of patients’ FND, behavioral health, psychosocial needs, and risk factors when appropriate.
- Develops individualized care plans in collaboration with patients, FND Program multidisciplinary team and outside care providers.
- Assists with formulating patient-centered goals that address FND recovery, mental health needs, social determinants of health, and barriers to care.
- Assists with overcoming barriers to attending visits in the FND Program.
- Coordinates with medical providers, mental health professionals, social workers, and other healthcare teams to implement care plans.
- Facilitates referrals to community resources, support services, and behavioral health programs (e.g., therapy, substance use treatment).
- Monitor patient progress and adjust care plans, as necessary, including follow up calls and filling out FMLA paperwork.
- Educates patients and families about available treatment options, coping strategies, and resources for managing FND.
- Advocate on behalf of patients to ensure they receive appropriate care, services, and support.
- Assist patients in accessing benefits, financial resources, and community services (e.g., housing, employment assistance).
- Ensures all care coordination activities comply with organizational policies, HIPAA, and other relevant regulations.
- Participates in quality and process improvement initiatives aimed at enhancing FND Program services and patient outcomes.
- Informs FND Program Director and Business Manager of behavioral health service line budgetary and resource needs and works to develop sustainable resource management database.
- Works closely with FND Program Clinic Coordinator to further develop and maintain statewide, FND Resource Collaborative.
- Collaborates with FND Program faculty and staff to support or conduct local and national educational engagement activities and initiatives.
- Participate in clinical research, quality, and process improvement initiatives; supervise trainees and support academic mission.
Requirements
- Principal Professional: Bachelor's degree from an accredited institution.
- Principal Professional: 3-5 years of experience in clinic case management, or highly related work.
- Senior Professional: Bachelor's degree from an accredited institution.
- Senior Professional: 2 years of experience in clinic case management, or highly related work.
- Professional: Bachelor's degree from an accredited institution.
- Professional: 1 year of experience in clinic case management, or highly related work.
- For all levels an advanced degree (Masters or Doctorate) may be substituted for experience on a year for year basis if the degree is in a field of study directly related to the work assignment.
- Prior experience working with families and caregivers.
- Must be able to work cooperatively with all constituencies in the UCHealth Hospital, Department of Psychiatry, and Department of Neurology including Departmental Chair(s), Program Directors, and administrative staff.
- Skills in risk assessment, including suicide and violence risk assessment. (Preferred)
- CITI Certifications will be arranged prior to appointment. (Preferred)
- Experience caring for functional neurological disorder (conversion disorder) or other somatic symptoms. (Preferred)
- Prior experience delivering trauma-informed care. (Preferred)
- Prior experience working in multidisciplinary teams. (Preferred)
- Comfortable with public speaking. (Preferred)
- Prior experience working within academic medical setting. (Preferred)
- Prior experience delivering virtual treatment. (Preferred)
- Familiar with federal and state programs and resources (e.g., Medicaid). (Preferred)
- Member of Functional Neurological Disorders Society and National Society of Social Work (Preferred)
