Provide high quality, effective care management to IEHP members ensuring coordinated continuous care
Conduct outreach and engagement, skilled therapeutic interactions, behavioral health interventions within scope, care coordination, and resource linkages
Utilize clinical expertise to support and engage Members to promote positive health behaviors and assist with coordination of care
Provide clinical expertise, clinical leadership, and clinical oversight; consult for non-licensed team members
Conduct comprehensive holistic assessments telephonically and in person (facility or home visits) and develop individualized care plans or discharge plans
Coordinate with internal and external health partners, participate in inter/transdisciplinary care team meetings, and facilitate transitions of care
Provide crisis intervention and clinical guidance; engage in proactive utilization and quality review of Behavioral Health services
Lead and provide clinical training and onboarding for department Team Members on behavioral health conditions and evidence-based treatments
Ensure documentation accuracy and compliance with regulatory requirements and accreditation standards
Assist Members with care coordination needs including PCP appointments, transportation, DME, and home health coordination
Requirements
Minimum of three (3) years of experience performing or facilitating Behavioral Health/Medical Social Work services
Master’s degree in Social Work or related field from an accredited institution required
Possession of an active, unrestricted, and unencumbered license in a Social Services related field issued by the California Board of Behavioral Sciences required (LCSW or LMFT preferred)
Experience in motivational interviewing and/or other evidenced-based communication strategies
Experience working successfully within a team and developing and maintaining effective relationships with clients and coworkers
Must have a valid California Driver's License
Behavioral Health/Medical Social Work services experience in a health clinic, psychiatric hospital, medical facility, or health care clinic strongly preferred
Experience in clinical services, both mental health and substance use preferred
Familiarity with providing Behavioral Health Care and discharge planning required
Bilingual (English/IEHP Threshold Language) – written and verbal is highly preferred
Proficient in Microsoft Word, Excel, PowerPoint; demonstrated proficiency with electronic medical management systems (e.g., Cisco, MHK/Care Prominence, MediTrac, SuperSearch and Web Portal) is preferred
Effective written and oral communication skills, reasoning and problem-solving skills, interpersonal communication and conflict resolution skills
Resiliency to tolerate and adapt to a moderate level of change and development around new models of care and care management practices
Ability to engage Members and providers on the phone as well as in person, prioritize multiple tasks, and work in a fast-paced environment
