Manager

• Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical and biological components, and appropriate nursing intervention and follow-up
• Coordinates the Plan of Care, ensuring that an individualized Plan of Care is developed that accurately reflects the patient’s evolving needs
• Educates patient, family, caregivers and other health professionals about disease process and decline, palliative interventions, care giving, dying process and safety practices
• Reports changes in the patient’s condition to appropriate members of the IDG or other health professionals

Per Diem PACE RN, Case Manager – PACE Program

Accountant

• The Staff Accountant is responsible for the day-to-day accounting functions such as: monitoring cash, preparing monthly journal entries, preparing monthly account reconciliations, assisting with annual audit, and performing other special projects related to financial management.
• Assists with reviewing on-line banking activity daily and updating the daily cash receipts log and monitoring positive pay.
• Assists with the preparation of the monthly: (1) bank reconciliations for all bank accounts (2) journal entries for assigned GL accounts and (3) account reconciliations and roll-forwards for assigned GL accounts.
• Reviews all capital expenditure requests for accuracy, submits for appropriate signature and updates the corresponding capital expenditure request log.
• Maintains fixed asset system; including records depreciation.
• Prepares the account reconciliations for all fixed asset accounts.
• Handles accounting for the prepaids and maintains the system prepaid module.
• Monitors and handles any necessary tasks related to the postage accounts.
• Performs other duties as assigned.

Staff Accountant

Counselor

• Provide individual and group counseling and therapy for Hospice patients, family members and community individuals who have experienced the loss of a significant person through grief.
• Demonstrate contact of all clients within established time frames and provide appropriate documentation.
• Document at least two additional attempts to reach an assigned client before closing the case.
• Demonstrate a willingness and ability to manage tasks and case loads assigned, from any departmental program, in an effective, productive and timely manner, meeting the standards that reflect expected productivity.
• Demonstrate a willingness to provide counseling services such as on call crisis intervention, individual and group, on a flexible schedule as needed, including some week nights, weekends and at camp.
• Document all client contacts and care plans accurately according to department and organization policy in a timely manner.

Bereavement Counselor

Therapist

• Conducts respiratory evaluation of patients as requested by clinical staff.
• Determines the need for utilization of respiratory equipment and supplies including BIPAP, CPAP, liquid oxygen, oxygen concentrators, nebulizers, oral and intermittent suction, continuous pulse oximetry and ventilators.
• Makes recommendations for respiratory treatments and care based on patient assessment to clinical and medical staff.
• Provides routine follow-up and monitoring.
• Evaluates treatment effectiveness, patient compliance and safety issues, and recommend changes.
• Provides education on respiratory therapy to clinical staff, medical staff and patient/caregiver education regarding respiratory equipment use and maintenance, respiratory medications and treatments, energy conservation, home safety and oxygen safety.
• Prepares and maintains required documentation for each assigned patient. Completes documentation in a timely manner and distributes to the appropriate parties.
• Performs other duties as assigned.

Respiratory Therapist

Collections

• Prepares and submits accurate billing to third party payor’s and self-pay invoices.
• Identifies and resolves billing clearinghouse rejections.
• Conducts root cause analysis on unpaid accounts receivable (AR) and resolves with third party payor’s.
• Analyzes and resolves credit balances.
• Handles incoming and outgoing customer service calls to third party payor’s, outside providers and patients/guarantors to resolve and achieve account resolution.
• Processes correspondence.
• Initiates and resolves technical denial appeals.
• Identifies payor denial and cash fluctuation trends and escalates as appropriate.

Frontend Engineer

• The Revenue Cycle Director provides operational leadership for Billing and Collections, and Insurance Verification and Authorization

Medical Billing and Coding

• The Hospice Coder is responsible for accurate and timely coding of hospice services in compliance with CMS regulations, ICD-10-CM, CPT, and HCPCS guidelines.
• This role plays a critical role in ensuring appropriate reimbursement, regulatory compliance, and high-quality patient care documentation.
• Analyzes and interprets information in the medical record and assigns the correct code(s) utilizing ICD-10-CM and or CPT-4 classification system to the diagnoses/procedures of medical records according to the coding guidelines.
• Abstracts all necessary information from medical records to identify the diagnosis and any related complications and co-existing conditions.
• Reviews medical staff documentation and assigns appropriate procedure codes including evaluation and management services.
• Reviews clinical documentation to ensure valid ICD-10-CM codes are assigned.
• Implements CHS physician query process when code assignments are not straight forward or documentation in the medical record is inadequate, ambiguous or unclear for coding purposes.
• Maintains a 95% coding accuracy rate as set by organization.
• Communicates with medical staff as needed to clarify documentation for appropriate code assignment.
• Evaluates medical record documentation in order to ensure the appropriate diagnoses and CPT codes are assigned to accurately reflect and support the visit, and to ensure that the information complies with regulatory standards and guidelines.
• Abides by the Standards of Ethical Coding as set forth by the American Health Information Management Association (AHIMA) and the American Academy of Professional Coders (AAPC) adhering to the official coding guidelines.
• Maintains knowledge of current coding guidelines and obtains continuing education units to maintain coding credentials.
• Demonstrates effective time management skills by completing assignments within time constraints and calendar schedule.

• Assists with evaluation and tracking of enterprise documents and contracts

Project Management Assistant

• Responsible for all staff and functions of Hope PACE.
• Analyze situations, problem solve and facilitate communications within the team and between other departments.
• Serve as a liaison between individual program disciplines, other PACE departments and community service providers.

Manager, Hope PACE Center – RN

• The RN, Case Manager is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Team (IDT), and providing clinical, palliative and supportive care to the patient/family unit in order to keep the participant in their home environment as long as possible.
• Provides reassurance on the phone to patients and families.
• Assists in finding solutions to their questions and/or recognizes the need for an in person visit.
• Coordinates in person visit when needed/or requested.
• Utilizes appropriate support/expert resources or personnel to resolve complex or difficult situations.
• Documents patient/family contact information in the EMR and communicates with the Interdisciplinary Team (IDT).
• Completes initial and semi-annual assessment for all Company services including, but not limited to: Explains services to patients/families and addresses questions regarding patient needs, fears, physical limitations, while putting the patient/family at ease; presents services in an empathetic and compassionate manner.
• Provides information to Physicians and other IDT members and initiates Plan of Care to address patient’s immediate needs.
• Initiates skilled nursing interventions to enhance prevention, prevent complications, alleviate symptoms and maximize physical and emotional comfort.
• Obtains Physician orders.
• Completes documentation per Company policy.
• Acts as the Company representative at assigned facilities while facilitating referrals to all service lines; works closely with referring hospitals, physicians, facilities, patients, families, and the general public.
