About the role
Key responsibilities & impact- Provides fully remote virtual serious illness care management to patients and families throughout our continuum of care.
- Consults with medical staff, and/or other members of the IDT as indicated to ensure optimal patient care in an effective and efficient manner.
- Performs virtual assessment of patient/family/caregiver needs.
- Identifies, manages, orders, and prescribes medication and equipment as appropriate.
- Maintains access to all electronic medical records (EMR), electronic prescribing systems, and other required websites and applications.
Requirements
What you’ll need- Must have a minimum of 3 years of recent clinical experience, including substantial experience in hospice, palliative care, and/or acute inpatient care.
- Must hold a Masters or Doctors degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
- The ReNW NP must hold national certification, licensure, and prescriptive authority from the appropriate governing body for advanced nursing practice in their home state and in additional state(s) where care is provided.
- Must hold and maintain Basic Life Support certificate from an approved organization and compact RN license in their home state by date of hire.
