LPN – Per Diem Continuous Care
Chapters Health SystemLPN providing continuous care and nursing interventions under RN supervision in healthcare settings. Ensuring patient comfort and enhancing quality of life for patients and their families.
About the role
Key responsibilities & impact- Under RN supervision, responsible for providing nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit
- Provides nursing tasks to patients and families such as continuous care, dressing changes, catheter changes as identified in the plan of care
- Demonstrates clinical skills regarding application of Company policies and procedures such as: medication administration, symptom management, and patient/family relationships
- Observes/monitors patients for alterations in body systems, pain and symptoms and reports to RN
- Reports findings to physicians, APRNs and physician’s assistants following collaboration with RN
- Performs venipuncture for blood specimens
- Documents per policy and procedure in medical record
- Performs other duties as assigned.
Requirements
What you’ll need- A high school diploma or G.E.D. or an equivalent combination of education and experience
- Current LPN licensure in the state of employment
- 6 months experience as LPN preferred
- Ability to communicate (written and verbal) in English
- Flexible schedule to meet the needs of patients
- Ability to manage the emotional stress of working with and caring for terminally ill patients and their families
- Ability to perform the essential functions and physical requirements of the job with or without reasonable accommodation
- Cross training to Hospice House positions of equal licensure/certification and scope when census calls for flexible staffing.
