About the role

Requirements
What you’ll need- Master’s degree or above in Social Work (CSWE accredited)
- Minimum of one (1) year of experience including social work/counseling internship in health care or hospital setting
- Familiarity with adult and pediatric patients' concepts
- Previous experience with an EMR/EHR system
- Mobile Driver - Valid driver's license and automobile insurance
- Ability to physically access homes and facilities
