About the role
- Grow hospice and home health admissions with clear, reachable goals
- Host patient/family info sessions to cut confusion and build trust
- Hand off referrals fast to clinical teams (no bottlenecks, no lost admits)
- Track and report results in CRM/EHR
- Educate the community on care options that actually help patients
- Support paperwork and signatures only when needed
- Own key referral growth from day one and collaborate with leadership to remove roadblocks
- Field-based work in Naperville/west suburbs with rare overnight travel (≤1x per quarter)
Requirements
- 1+ year hospice or home health marketing experience
- Active driver’s license and reliable insured vehicle
- Clear driving record
- Strong written and verbal communication skills
- Comfort with Microsoft Office and EHR systems
