About the role

  • Conduct field-based outreach, including home visits and doorknocking, to engage patients and explain available services
  • Make outbound calls to prospective and current patients, effectively communicating the benefits of Synapticure’s programs and addressing questions
  • Build and maintain relationships with physician offices and healthcare providers to facilitate referrals and improve care coordination
  • Educate patients and caregivers about program benefits, addressing concerns with empathy and clarity
  • Identify and address barriers to patient engagement, developing creative solutions to encourage enrollment and participation
  • Maintain accurate, timely documentation of outreach activities, patient interactions, and outcomes in designated systems
  • Collaborate with internal teams, including care coordination and clinical staff, to ensure seamless patient support
  • Adhere to HIPAA and company policies to protect patient privacy and data security
  • Participate in team meetings, trainings, and continuous improvement initiatives
  • Represent Synapticure in the community, visiting patient homes and physician offices to increase enrollment and retention in services

Requirements

  • High school diploma or equivalent required; Associate’s or Bachelor’s degree in healthcare administration, social work, marketing, or related field preferred
  • Minimum 2 years of experience in patient engagement, community outreach, sales, or a related healthcare role
  • Proven experience conducting both in-person and telephone-based patient outreach
  • Ability to work independently in a field-based role, managing your own schedule and territory
  • Exceptional interpersonal and communication skills with the ability to quickly build rapport with diverse populations
  • Empathetic listener with a patient-centered approach to engagement
  • Strong organizational skills and attention to detail for accurate documentation
  • Resourceful problem-solver, adaptable to varying patient needs and situations
  • Proficiency with CRM systems, EHR platforms, or other patient management software
  • Flexibility to work evenings or weekends as needed to accommodate patient availability
  • Active, valid driver’s license, current registration, insurance, and ability to drive up to five days per week in the service area
  • Knowledge of healthcare systems, patient rights, and HIPAA regulations
  • Bilingual proficiency (Spanish strongly preferred)
  • Experience with risk adjustment or managed care populations (preferred)
  • Familiarity with local community resources and healthcare networks (preferred)
