About the role
- Provide professional, comprehensive patient centric care management services for at-risk patients in an acute care environment
- Perform assessment and planning, coordination of care, discharge planning, and documentation of interventions
- Ensure regulatory compliance and act as patient advocate
- Identify needed resources to address social, financial, cognitive/behavioral or legal barriers to care access
- Ensure continuity of care for vulnerable patients by coordinating community and clinical resources
- Collaborate with interdisciplinary clinical teams to support patient transitions and discharge planning
Requirements
- Master's Degree in Social work from an accredited CSWE program
- Upon hire: Texas Masters Social Worker License - Texas State Board of Social Worker Examiners
- 1 or more years of Social work experience in any healthcare setting
- 1 or more years of experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program
- Basic understanding of acute medical surgical healthcare terminology (preferred)
- Prior case management experience (preferred)
- Must reside within commutable distance of the following office location: Texas: Lubbock
- Must be available to work weekends and holidays as scheduled
- This is an onsite position, requiring daily attendance at the designated location
