About the role
- Work remotely while providing in-person housing support for patients as needed in the Inland Empire area
- Assist patients in securing and maintaining stable housing, including support with housing applications, financial assistance programs, and landlord negotiations
- Connect patients with local, state, and federal housing resources (HUD programs, Section 8, emergency rental assistance)
- Integrate housing support into care plans by collaborating with healthcare teams, case managers, and social workers
- Educate patients on financial and economic resources to improve overall health and stability
- Provide regular updates to the care team regarding patient needs, barriers, and progress
- Focus on enhancing patient well-being and addressing housing-related healthcare disparities for medically complex patients
Requirements
- Associate’s Degree (required); Bachelor’s Degree preferred
- 2+ years of experience in housing support services (required)
- Strong knowledge of federal and state programs (e.g., HUD, Section 8, emergency rental assistance, disability benefits, food assistance, IHSS)
- Experience working with medically complex patients and care management
- Ability to provide in-person housing support, including housing applications, financial assistance navigation, and landlord negotiations
