About the role
- Provide high quality nursing care to patients in the comfort of their own homes
- Perform assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting
- After visits, complete documentation, make calls, attend meetings, coordinate patient care and arrange visit schedules
- Serve as a primary coordinator of all disciplines for well-coordinated patient care
- Perform accurate and timely admission assessments and reassessments within specified time frames
- Assess patients for safety risks, abuse and neglect and manage patients' pain
- Coordinate with interdisciplinary teams and community resources to support patient needs
- Participate in Cleveland Clinic Home Care orientation and ongoing professional development
Requirements
- Graduate from an accredited school of Professional Nursing
- Current state licensure as a Registered Nurse (RN)
- Basic Life Support (BLS) Certification through the American Heart Association (AHA)
- Valid Ohio Driver's (VDL) license upon hire and per policy
- Proof of $100,000/$300,000 automobile insurance coverage upon hire and per policy
- One year of current experience as an RN
- One year of experience in an acute Medical/Surgical or Home Care setting
- Preferred: Bachelor’s of Science in Nursing (BSN)
- Preferred: Experience working in homecare, med/surg, ICU and/or ED areas
- Ability to perform accurate and complete admission assessments and reassessments within specified time frames
- Ability to assess patients for safety risks, abuse and neglect
- Ability to assess, document, prioritize and manage patients’ pain
- Must be ambulatory and able to drive to patients' homes, climb stairs, pull, push, facilitate patient movement
- Requires standing and walking for extensive periods, manual dexterity, corrected hearing and vision
- Must pass controlled substance and nicotine testing and comply with influenza vaccination program
