Interprets physician referrals and conducts speech and language evaluations for patients with various speech, language, voice, cognitive and swallowing disorders
Establishes and implements individualized plans of care and treatment based on evaluation results and functional goals
Monitors and evaluates outcomes, adjusts treatment plans, and provides status updates to physicians
Plans and completes patient discharge including written home programs, patient/family teaching, community referrals, and ordering equipment
Completes required documentation including progress notes, team conference reports, discharge summaries, statistics, billing, and scheduling
Applies insurance guidelines regarding authorization and number of visits
Cleans up after patient treatment, maintains equipment and area
Performs manual techniques, patient positioning, and transporting duties; uses modalities and exercise equipment per department orientation
Participates in performance improvement activities and manages assigned caseload
Pursues professional growth, orients new staff, and participates in co-worker evaluations as requested
Requirements
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required
Speech-Language Pathology license issued by the state of Wisconsin
Master's Degree in Speech Pathology
No experience required
Intermediate computer skills

Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served; knowledge of growth and development over the life span
Team members will maintain annual educational requirements for hospital based certifications (e.g.DNV, CARF or others)
Physical requirements: sit, stand, walk, lift, squat; lift up to 50 lbs from floor to waist; lift up to 10 lbs from waist to overhead; lift and carry up to 40 lbs at waist height; push/pull with 30 lbs of force; perform a sliding transfer of 150 lbs with a second person present; repetitive use of hands; functional use of senses; ability to wear protective clothing; position may require travel
