About the role
- Interprets physician referrals and conducts speech and language evaluations for patients with various speech, language, voice, cognitive and swallowing disorders
- Establishes and implements individualized plans of care and treatment based on evaluation results and functional goals
- Monitors and evaluates outcomes, adjusts treatment plans, and provides status updates to physicians
- Plans and completes patient discharge including written home programs, patient/family teaching, community referrals, and ordering equipment
- Completes required documentation including progress notes, team conference reports, discharge summaries, statistics, billing, and scheduling
- Applies insurance guidelines regarding authorization and number of visits
- Cleans up after patient treatment, maintains equipment and area
- Performs manual techniques, patient positioning, and transporting duties; uses modalities and exercise equipment per department orientation
- Participates in performance improvement activities and manages assigned caseload
- Pursues professional growth, orients new staff, and participates in co-worker evaluations as requested
Requirements
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required
- Speech-Language Pathology license issued by the state of Wisconsin
- Master's Degree in Speech Pathology
- No experience required
- Intermediate computer skills
- Excellent communication and interpersonal skills
- Excellent organization, decision-making, and problem solving skills
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served; knowledge of growth and development over the life span
- Team members will maintain annual educational requirements for hospital based certifications (e.g.DNV, CARF or others)
- Physical requirements: sit, stand, walk, lift, squat; lift up to 50 lbs from floor to waist; lift up to 10 lbs from waist to overhead; lift and carry up to 40 lbs at waist height; push/pull with 30 lbs of force; perform a sliding transfer of 150 lbs with a second person present; repetitive use of hands; functional use of senses; ability to wear protective clothing; position may require travel
