About the role
- Provide high quality nursing care to patients in the comfort of their own homes
- Provide assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting
- After seeing patients, work remotely to complete documentation, make calls, attend meetings, coordinate patient care and arrange visit schedules
- Provide direct nursing care and perform accurate and complete admission assessments and reassessments within a specified time frame
- Assess patients for safety risks, abuse and neglect and serve as a primary coordinator of all disciplines for well-coordinated patient care
- Assess, document, prioritize and manage patients’ pain
- Manage patient assignments autonomously and make critical decisions in the field
- Attend to requirements for weekends, holidays and some on call
Requirements
- Graduate from an accredited school of Professional Nursing
- Current state licensure as a Registered Nurse (RN)
- Basic Life Support (BLS) Certification through the American Heart Association (AHA)
- Valid Ohio Driver's (VDL) license upon hire and per policy 8-175 thereafter
- Proof of $100,000/$300,000 automobile insurance coverage upon hire and per policy 8-175 thereafter
- One year of current experience as an RN
- One year of experience in an acute Medical/Surgical or Home Care setting
- Preferred: Bachelor’s of Science in Nursing (BSN)
- Preferred: Experience working in homecare, med/surg, ICU and/or ED areas
