• Manage a pipeline of families who have previously engaged with A Place for Mom and require continued guidance toward a move-in decision.
• Execute consistent, high-volume outreach across phone, text, and email to re-engage and advance families.
• Drive outcomes by moving families from tour to decision, including scheduling additional tours and providing new referrals when needed.
• Apply a structured follow-up cadence to maintain engagement and prevent pipeline stagnation.
• Build trust quickly with families, understand their needs, and guide them to the best-fit care options.

• Own end-to-end product design for Consumer workstreams, from discovery and journey mapping through prototypes and production-ready design, including the unified family experience across APFM.com, AgingCare, Care Hub, advisor-connected workflows, and emerging AI-enabled channels.
• Translate emotionally complex, information-heavy senior care decisions into intuitive workflows that help families move forward with confidence.
• Partner with Product, Engineering, Research, Marketing, Analytics, Operations, Sales, and business leaders to define problem framing, success criteria, tradeoffs, and release scope.
• Design for trust, clarity, accessibility, speed, and usability across moments where families need accurate information and confident next steps.
• Use research, analytics, care-seeker conversations, marketing insights, funnel data, frontline team input, product data, and business context to guide design decisions.
• Identify high-leverage opportunities where better consumer experiences can improve family outcomes, provider outcomes, and business performance.
• Communicate design rationale clearly to product teams, engineering partners, marketing partners, operational stakeholders, and senior leaders.
• Help teams understand not only what should ship, but why it matters.
• Apply and extend shared APFM patterns so consumer product surfaces feel coherent across channels and touchpoints.
• Mentor and raise the bar for other designers through critique, pattern quality, and strong product storytelling, without requiring people-management responsibility.

Lead Product Designer, Family Experience

User Researcher

• Independently own generative and evaluative research across Consumer and Provider experiences, helping teams understand APFM's two core customers: families navigating care decisions and providers growing their businesses while delivering care.
• Shape and maintain a practical research plan tied to product strategy, design priorities, marketplace questions, and leadership decisions.
• Plan and run interviews, usability studies, concept tests, journey research, surveys, diary or longitudinal studies when appropriate, and lightweight mixed-method studies.
• Help teams understand families navigating senior care decisions and providers managing lead quality, engagement, and conversion.
• Synthesize findings into crisp, decision-oriented artifacts that influence roadmap, product and user experience direction, service design, and product strategy, connecting research to the broader care placement journey where family needs, provider needs, advisor workflows, AI capabilities, and business outcomes intersect.
• Partner with Product, Design, Analytics, Engineering, Marketing, Operations, Sales, and customer-facing teams to connect qualitative insight with behavioral, funnel, marketplace, and business data.
• Identify cross-squad patterns, unmet needs, trust gaps, high-risk assumptions, and experience risks before they become delivery problems.
• Facilitate conversations that help teams align on what we know, what we need to learn, and what decisions should change.
• Contribute to repeatable research practices so teams can learn faster without lowering the quality bar.
• Use and improve lightweight rituals, templates, participant pipelines, and insight repositories that make Consumer and Provider insight easier to reuse.
• Partner with designers and product managers on practical research planning, method selection, and evidence-backed decisions.
• Model strong research craft and help teams choose the right level of rigor for the question, from rapid directional learning to deeper strategic research.

• Help APFM evaluate AI-enabled product experiences for trust, usefulness, clarity, safety, adoption, and edge cases in moments where families and providers need confidence.

• Identify high-leverage opportunities where better provider workflows can improve customer outcomes and business performance.
• Communicate design rationale clearly to product teams, engineering partners, marketing partners, operational stakeholders, and senior leaders.
• Help teams understand not only what should ship, but why it matters.

Lead Product Designer, Provider Experience

• Lead, manage, and develop a high performing, results-driven sales team via recruitment, training, coaching, and motivating
• Provide strong, dynamic leadership that mentors, develops, and guides team members to efficiently grow each account and drive the most referrals, move-ins, and new clients to our community and agency customers
• Train, mentor and manage team on how to support families being discharged by understanding each family’s unique needs and working with Senior Living Advisors to refer to the appropriate providers
• Partner with teams across the business to roll out new initiatives, pilots, or processes (e.g., account scoring, CRM changes) to your territory to enhance team productivity
• Manage performance of the team to maximize outcomes
• Achieve and exceed sales goals monthly to achieve company goals
• Prepare and report on market performance to Senior Leadership as required
• Occasional travel required for Divisional and Company sales meetings
• Complete customer experience calls monthly for each team member to ensure customer satisfaction and solve any potential service recovery issues
• The position will require a flexible work schedule between the hours of 8am and 8pm, with night and weekend call requirements and ~75% of time in-field (regional travel) with your team

Regional Sales Director – Healthcare

• Work in a fast-paced, growing industry to help families and professional referral sources with seniors urgently needing to be discharged into a community meeting their needs or with a home care agency as they transition out of a hospital or skilled nursing facility
• Deliver on your target quota of families selecting a senior care option each month by generating daily qualified patient referrals from your assigned market plan accounts
• Currently maintains a portfolio of clients (social worker, case managers, and discharger planners) specifically in hospitals and skilled nursing centers within this open territory
• Develop, own, and grow your book of accounts to increase referral volume in your territory
• Cultivate new contacts within each account through networking, cold calls, and service presentations, following up with each referral source on discharge outcomes to reinforce the value A Place for Mom delivers
• Educate families on their care options and how they will work with you and a Healthcare Senior Living Advisor to find the right senior living option as they are discharged
• Work with your partner Healthcare Senior Living Advisors to deliver on your target quota of families in moving into a community or hiring in home care
• Leverage and analyze reports in our CRM and internal structure to develop and manage your pipeline
• Pilot new initiatives, tests, and processes (e.g., account scoring, CRM changes) in your territory and provide feedback to improve the tools and resources at your disposal

Healthcare Account Executive

• Lead, manage, and develop a high performing, results-driven sales team via training, coaching, and motivating
• Conduct consistent in-field coaching with Healthcare Account Executives including ride-alongs, account strategy reviews, and live sales observation~75% of time in-field (regional travel - from Boston to Washington, D.C.) with your team
• Provide strong, dynamic leadership that mentors, develops, and guides team members to efficiently grow each account and drive the most referrals, move-ins, and new clients to our community and agency customers
