• Lead all Community Health Worker operations across markets, including workforce planning, deployment, productivity, and performance management

Director, CHW Operations

Customer Support

• Conduct outbound and inbound calls to potential patients to explain services and assist with enrollment or scheduling.
• Complete short intake forms to accurately collect demographic, eligibility, and clinical information.
• Schedule patient appointments based on availability, location, and provider match.
• Meet or exceed daily, weekly, and monthly performance goals (e.g., calls made, conversions, appointments scheduled).

• Convert leads to patients using internal documentation systems.
• Document all outreach attempts and outcomes in Athena and the appropriate trackers.
• Assist in sending patient mailers and making follow-up calls to leads generated from community events to enroll them in programming.
• Represent Zócalo Health at community coalition meetings and collaborative events.

Seasonal Community Engagement Specialist

Community Engagement Specialist

General

• Our care model is designed to meet members wherever they are—whether in their homes, online, or in their community. Care Coordinators play a critical role in ensuring members receive timely access to healthcare services, community resources, and care team support.
• Zócalo Health is seeking a Care Coordinator to support members enrolled in Enhanced Care Management (ECM) and other care coordination programs. The Care Coordinator serves as the primary point of contact for low-risk members and provides ongoing outreach, appointment coordination, referral management, and navigation support. Working closely with Registered Nurse Care Managers, Behavioral Health Care Managers, Community Health Workers, providers, and community partners, the Care Coordinator helps ensure members remain engaged in care and connected to the resources they need.
• This position reports to the Care Coordination Manager and works closely with the interdisciplinary care team.

Care Coordinator

Manager

• Conduct comprehensive clinical assessments and identify medical, behavioral, and social barriers to health.
• Develop, implement, and monitor individualized care plans for high-risk members.
• Manage a panel of members with complex healthcare needs across multiple settings and providers.
• Coordinate transitions of care following emergency department visits, hospitalizations, and skilled nursing stays.
• Collaborate closely with Community Health Workers to ensure members receive both clinical and community-based support.
• Identify gaps in care and proactively intervene to prevent avoidable utilization and adverse outcomes.
• Provide health coaching, disease management education, medication support, and self-management training.
• Participate in interdisciplinary care team meetings and case reviews.
• Maintain accurate documentation and regulatory compliance requirements associated with ECM and related programs.
• Support quality, utilization, and member outcome goals established by the organization and health plan partners.

RN Case Manager

Data Engineer

• Build and operate production-grade ingestion pipelines from core clinical, operational, and third-party systems into our Databricks lakehouse
• Develop and maintain dbt models that turn raw data into clean, well-documented, analytics-ready datasets
• Establish data quality, testing, and monitoring practices that make pipelines reliable and trustworthy
• Help shape ingestion patterns and architecture standards alongside the Principal Data Engineer
• Enable company-wide metrics for care outcomes and operations
• Collaborate with cross-functional leads to develop and iterate on a suite of core operational dashboards, ensuring teams have the self-service tools they need to track company metrics and outcomes.
• Design, build, and operate production data pipelines across clinical, operational, and third-party systems using API-based ingestion, Change Data Capture (CDC), and event- or webhook-driven patterns
• Build and maintain transformation layers in dbt, including tests, documentation, and reusable models
• Develop and refine core analytical and longitudinal data models used across the company
• Implement testing, monitoring, and observability to ensure data quality, pipeline reliability, and system performance
• Apply strong engineering fundamentals to improve the scalability, performance, and cost-efficiency of data systems on AWS and Databricks
• Partner with Product to support metric definitions, outcome measurement, and reporting needs
• Contribute to engineering standards, code review, and a culture of knowledge sharing and continuous improvement
• Partner with business, product, and engineering stakeholders to design and build intuitive data visualizations and dashboards that drive actionable insights and program visibility.
