Director, CHW Operations
Zócalo HealthDirector of CHW Operations leading community health initiatives at Zócalo Health. Responsible for building and scaling Community Health Worker operations across multiple markets while ensuring quality and integration.
About the role
Key responsibilities & impact- Lead all Community Health Worker operations across markets, including workforce planning, deployment, productivity, and performance management
- Establish consistent standards for home visits, community-based interventions, member engagement, outreach, and field documentation
- Build management systems that support coaching, accountability, quality, and professional development across the CHW organization
- Partner closely with Nurse Practitioners, RN Care Managers, Behavioral Health Case Managers, and Care Coordinators to ensure seamless integration within care teams
- Monitor operational performance and identify opportunities to improve member engagement, retention, and outcomes
- Support expansion into new markets while maintaining consistency of service delivery and operational excellence
- Serve as the primary leader responsible for the effectiveness and scalability of the CHW function
Requirements
What you’ll need- 8+ years of healthcare operations leadership experience
- Experience leading Community Health Workers, care coordinators, patient navigators, outreach teams, or other community-based healthcare workforces
- Experience building and managing distributed field teams
- Strong understanding of Medicaid populations and social determinants of health
- Demonstrated success developing frontline leaders and improving operational performance
- Excellent communication, coaching, and change management skills
