Child and Adolescent Mental Health Case Manager I – Bilingual, English/Spanish

• coordinate mental health and other health related services to improve or maintain child/student’s emotional and behavioral functioning
• help families access needed resources for the child including obtaining or maintaining third-party insurance or Medical Assistance coverage
• complete timely documentation and planning including Individual Family Community Support Plans, Progress Notes, case management authorizations, and treatment plans etc.
• educate and support families to become empowered to better self-advocate and navigate the various systems with more confidence and skill
• complete a functional and diagnostic assessment of the medical, social, educational, and other services and resources the child will need to address their mental health needs.
• develop an Individual Family Community Support Plan (IFCSP) based on the functional assessment and diagnosis.

Child and Adolescent Mental Health Case Manager II – Bilingual, English/Spanish

Therapist

• Provide intensive support to children and adolescents temporarily staying at Children's Minnesota Hospital.
• Carry a caseload of 5–10 clients and deliver culturally responsive, wraparound care.
• Services may include assessment, crisis intervention, therapy, parent education, and skills training.
• Help children and families transition safely from the hospital back into the community.
• Work collaboratively with families and providers to create safety plans and coordinate ongoing care.

Acute Response Therapist II

• Provide a combination of mental health services, including:
• Group Therapy – Provide group therapy, including conducting Diagnostic Assessments, developing and implementing individualized treatment plans, and leading scheduled group services
• Individual Therapy – Provide individual therapy to clients, conducting Diagnostic Assessments and developing individualized treatment plans
• Family Therapy – Provide family therapy with the client’s family, conducting Diagnostic Assessments and developing individualized treatment plans
• Maintain required documentation as specified in Clinical Policy Manual

Acute Response Therapist II – Children’s Hospital

Community Manager

• Ensure clinicians provide effective assessment and therapeutic services: provide training as needed to support crisis and DBT clinicians; collaborate with intake to ensure referrals and requests for services are triaged for program fit, responded to, and processed efficiently and effectively; work with clinical, support, and reimbursement staff and external resources to refine systems to improve access and facilitate enrollment with oversight from the Clinical Director; coordinate afterhours coverage services and ensure all clients have active safety plans; support staff professional development related to culturally responsive clinical practice; maintain knowledge of and supervise staff ensuring practice is consistent with the Agency’s Clinical Policies and Procedures; provide regular clinical supervision and be available for clinicians needing quick and timely case consultations.
• Oversee clinical documentation practices to meet state and contract requirements: review charts, sign-off, and discuss assessments, treatment plans, progress notes, clinical supervision, and other clinical documentation for prelicensed clinicians and staff needing support; ensure MN and HIPPA data privacy requirements are followed.
• Outcomes collection: work with staff to reliably collect outcomes data and use for consultation, evaluation, quality improvement, and reporting purposes.
• Supervision and Leadership: recruit, interview, and hire qualified staff; oversee supervision of interns; complete timely annual and quarterly performance evaluations; facilitate weekly team meetings and monthly consultation groups; provide weekly individual and/or group supervision; provide resources, training, feedback, and activities to build a cohesive team; create a positive, respectful work environment; provide indirect evaluation and supervision of support staff; collaborate with clinical supervisors and internal administrative teams for continuous improvement; build learning and succession plans.
• Teamwork and Cooperation: participate in Agency committees and teams; meet regularly with the Clinical Director; participate in external meetings with providers, referral sources, payers, and stakeholders; actively participate with the clinical supervisors team including work groups, planning retreats and trainings.
• Program Management: manage and oversee DBT program activities in coordination with the Director of Community-Based Programs; provide oversight of DBT Skills Group co-facilitators and sign off on paperwork when required; coordinate and implement weekly DBT consultation; ensure clients receive services consistent with the program model; coordinate program intakes and enrollments; facilitate assignment of clients and manage caseloads; work with program clinicians and reimbursement staff to maintain timely authorizations for services.
• Financial and Reporting Oversight: participate in program budget development and monitor revenue and expenses; review utilization and financial reports and develop follow-up plans; work with Clinical Director and HR to identify program expansion opportunities; complete required reporting (e.g., Hennepin County outcomes reports, grant reports); ensure compliance with legal, ethical, and licensing guidelines (e.g., Rule 29, DHS Provider Manual Chapter 16, grievance logs, data privacy).
• Professional Development and Training: ensure new staff complete required trainings; lead team diversity and cultural responsiveness efforts; participate in planning Agency-wide and team training events; support arrangements for new staff and interns to attend Agency trainings.
• Individual CSE expectations: facilitate English-speaking DBT skills groups when necessary; provide oversight on required paperwork for DBT clients; prepare and submit insurance prior authorizations when required; provide individual DBT therapy when necessary; participate in 1.5 hours of consultation with the DBT team each week; maintain documentation according to agency timeliness expectations.
