• Provide intensive support to children and adolescents temporarily staying at Children's Minnesota Hospital.
• Carry a caseload of 5–10 clients and deliver culturally responsive, wraparound care.
• Services may include assessment, crisis intervention, therapy, parent education, and skills training.
• Help children and families transition safely from the hospital back into the community.
• Work collaboratively with families and providers to create safety plans and coordinate ongoing care.
