managing a caseload of 17-18 children or youth and their families with all client service expectations that accompanies this caseload.
provide culturally based, individualized targeted Children’s Mental Health Case Management to children/students and families in their home, schools and community based on each family’s needs.
coordinate mental health and other health related services to help the family access needed resources for the child including obtaining or maintaining third-party insurance or Medical Assistance coverage.
complete timely documentation and planning including Individual Family Community Support Plans, Progress Notes and case management authorizations.
provide services to educate and support families to become empowered to better self-advocate and navigate various systems with more confidence and skill.
Requirements
A bachelor's degree in one of the behavioral sciences or a related field including, but not limited to: social work, psychology, human services, or sociology from an accredited college or university
An LSW (License of Social Work) is required, should you have a BSW
Fluency in both English and Spanish
1 year of experience with mental health compromised children and families
Valid Driver’s License and access to reliable transportation is required
A successful candidate should be able to demonstrate knowledge of cultural diversity and the role of culture in the therapeutic process
A flexible, resourceful approach to the work with experience or in-depth knowledge of home and community-based services
