About the role
- managing a caseload of 17-18 children or youth and their families with all client service expectations that accompanies this caseload.
- provide culturally based, individualized targeted Children’s Mental Health Case Management to children/students and families in their home, schools and community based on each family’s needs.
- coordinate mental health and other health related services to help the family access needed resources for the child including obtaining or maintaining third-party insurance or Medical Assistance coverage.
- complete timely documentation and planning including Individual Family Community Support Plans, Progress Notes and case management authorizations.
- provide services to educate and support families to become empowered to better self-advocate and navigate various systems with more confidence and skill.
Requirements
- A bachelor's degree in one of the behavioral sciences or a related field including, but not limited to: social work, psychology, human services, or sociology from an accredited college or university
- An LSW (License of Social Work) is required, should you have a BSW
- Fluency in both English and Spanish
- 1 year of experience with mental health compromised children and families
- Valid Driver’s License and access to reliable transportation is required
- A successful candidate should be able to demonstrate knowledge of cultural diversity and the role of culture in the therapeutic process
- A flexible, resourceful approach to the work with experience or in-depth knowledge of home and community-based services
