Lead Care Manager role: serves as client's primary point of contact; coordinates with providers, manages client cases, coordinates health care benefits, provides education and facilitates access to care; develops and oversees client care plans and SMART goals; offers services in-office, telehealth, or field-based; connects clients to social services and supports; advocates with health care professionals; utilizes Motivational Interviewing, Harm Reduction, Trauma-Informed Care; conducts outreach and engagement and logs activity in CRM; evaluates client progress and updates goals; provides mental health promotion; arranges transportation; completes documentation and monthly reporting; maintains EMR and attends training.
Requirements
Willing and able to work Monday-Friday 8:30am-5:00pm, both in the field and remotely.
2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations
Working knowledge of government and community resources related to social determinants of health
Clean driving record, valid driver's license, and reliable transportation
Excellent oral and written communication skills
Positive interpersonal skills required
Must have general computer skills and a working knowledge of Google Workspace, MS Office and the internet
Bilingual (English/Spanish) preferred
Legally authorized to work in the United States (E-Verify upon hire)
Background check and possible drug/health screenings required
Vaccination against COVID-19 and influenza required for patient-facing roles (accommodations considered)
