Lead Care Manager at Vynca serving as the client’s primary contact; coordinates care with providers to align on client needs and care; manages client cases and coordinates benefits; educates and facilitates access to care; collaborates with caregivers; promotes wellness and member empowerment; this is a hybrid role that requires traveling throughout the San Joaquin County area.
What you’ll do: assess member needs across physical/mental health, SUD, oral health, palliative and memory care, trauma-informed care, social supports, housing; develop care plans and goals; offer services where the member is; connect to social services; advocate with healthcare professionals; use Motivational Interviewing, Harm Reduction, Trauma-Informed Care; engage with CRM; evaluate progress and update SMART goals; provide mental health promotion; arrange transportation; complete documentation; maintain EMR records; complete monthly reporting; attend training.
Your experience and qualifications: Monday-Friday 8:30am-5:00pm; 2+ years of care management experience; knowledge of social determinants of health; clean driving record and reliable transportation; strong communication; interpersonal skills; computer skills; bilingual English/Spanish is a plus
Requirements
Willing and able to work Monday-Friday 8:30am-5:00pm, both in the field and remotely.
2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations
Working knowledge of government and community resources related to social determinants of health
Clean driving record, valid driver's license, and reliable transportation
Excellent oral and written communication skills
Positive interpersonal skills required
Must have general computer skills and a working knowledge of Google Workspace, MS Office and the internet
Bilingual (English/Spanish) is a plus
