About the role
- Lead Care Manager serving as primary contact and coordinating care among providers to ensure client needs are met
- Manage client cases, coordinate healthcare benefits, provide education and facilitate timely, cost-effective access to care
- Assess member needs across physical, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community services
- Oversee development of client care plans and SMART goals; evaluate progress and update goals
- Offer services in-office, via telehealth, or field-based where member resides or seeks care
- Connect clients to social services and supports; arrange transportation (e.g., ACCESS)
- Advocate with healthcare professionals and utilize evidence-based practices like Motivational Interviewing, Harm Reduction, Trauma-Informed Care
- Conduct outreach and engagement; log activity in CRM and maintain EMR and business system records
- Complete documentation and monthly reporting; attend assigned training
- Hybrid role requiring travel throughout Santa Clara County and field work
Requirements
- Willing and able to work Monday-Friday 8:30am-5:00pm, both in the field and remotely.
- 2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations
- Working knowledge of government and community resources related to social determinants of health
- Clean driving record, valid driver's license, and reliable transportation
- Excellent oral and written communication skills
- Positive interpersonal skills required
- General computer skills and working knowledge of Google Workspace, MS Office and the internet
- Bilingual English/Spanish is a plus
- Background check (may include drug/health screenings) required prior to employment
- COVID-19 and influenza vaccination required for patient/client-facing roles (accommodations considered)
