About the role

  • Clinical oversight and management of ECM Lead Care Managers to address medical, behavioral health, and social determinants of health.
  • Communicate with ECM Lead Care Managers and occasionally with patients via telephone, telehealth, or interdisciplinary care team meetings regarding medication management and follow-up.
  • Serve as escalation point and guide crises encountered by Lead Care Managers.
  • Collaborate with broader care team (PCP, BH clinicians, specialists) to advocate for clients and ensure needed services.
  • Monitor team progress, coach Lead Care Managers, and manage performance as needed.
  • Supervise approximately ten Lead Care Managers and oversee creation of Individual Care Plans.
  • Drive the team to meet key performance indicators (KPIs) and implement process improvements with the Director of Enhanced Care Management.
  • Provide direct care management as needed to support organizational goals.
  • Hybrid role requiring travel throughout Sacramento County up to 2-3x per month.
  • Role is critical and hiring is urgent.

Requirements

  • Availability to work Monday-Friday, 8:30 am-5:00 pm (Pacific Time) both in the field and remotely, with flexibility for potential evenings and weekends.
  • Active RN license in California; willingness to obtain additional state licensure as needed.
  • 2 years of people management experience (4 years preferred).
  • 2-3 years of care management experience.
  • 1-2 years of post-acute experience.
  • Proven, results-driven leader with experience in change management in rapid-growth environments.
  • Strong organizational and independent working skills.
  • Proficient in Microsoft and Google applications; Salesforce experience is a plus.
  • Excellent communication and motivational skills, including conflict resolution techniques.
  • Working knowledge of lean Six Sigma methodology is preferred.
  • Background check and possible drug/health screening required prior to employment.
  • Vaccination requirement: COVID-19 and influenza vaccination required for patient/client-facing roles (accommodations considered).
