About the role
- Works independently and as part of a specialty medical team to provide nursing services including administration of medications and injections, immunizations, referrals, and health instruction.
- Conduct health assessments, provide brief interventions, triage patients and support navigation of health and social services systems.
- Administer medical care and guidance in unhoused encampments and in TVHC's provided van.
- Prioritize individuals with high medical and behavioral health needs and/or Substance Use Disorders for intensive visits to improve health and lift them out of homelessness.
- Serve as patient advocate: assess learning needs, develop and implement patient education, act as resource and contact person.
- Respond to immediate survival needs prioritized by clients (basic needs, problem-solving, health education, community services information).
- Provide medical oversight of Community Health Worker and Behavioral Health Case Manager.
- Provide linkage and referrals to community services and work from an interdisciplinary approach.
- Represent TVHC at county and community meetings and engage with community stakeholders.
- Participate in multidisciplinary staff meetings for program development, staff development, program evaluation, and operations review.
- Maintain confidentiality of privileged administrative and patient information and complete required documentation in a timely manner.
- Provide outreach and engagement, health services and triage, and collaborate with Housing Resource Centers.
- Build relationships over time via consistent visits with potential patients and conduct regular outreach to build trust.
- Screen and identify individuals with medical and behavioral needs and participate in regular meetings with Alameda County Healthcare for the Homeless.
- Provide culturally competent, trauma-informed, and harm reduction-based services.
- Perform all duties and services in full compliance with TVHC’s Service Excellence Standards.
Requirements
- Current, valid California RN license.
- Current, Valid Cardio Pulmonary Resuscitation Card (CPR).
- Valid CA driver’s license and insurance.
- Preferably, two years of nursing experience serving patients with complex needs and ideally, in unsheltered populations.
- Bilingual in English, Spanish, or other languages is preferred.
- Ability to work independently, organize tasks, problem-solve, make decisions, work collaboratively as a team member, and handle multiple priorities.
- Demonstrated experience with quality improvement processes to improve patient care and meet required health risk outcome measures.
- Culturally sensitive and demonstrated ability and effectiveness working with ethnically diverse populations.
- Sensitivity to and understanding of the special needs of unsheltered people facing complex challenges.
- Ability to effectively intervene in crisis situations with de-escalation techniques.
- Desire to outreach to unsheltered community members.
- Excellent communication and documentation skills at level necessary for taking patients’ medical histories, and accurately documenting patients’ medical information.
- Ability to effectively communicate with patient population and staff while demonstrating a high degree of diplomacy and tact.
- Ability to multi-task and work effectively in a high-stress and fast-moving environment.
- Possess a thorough understanding of the importance of confidentiality and non-disclosure according to the general standards set forth by HIPAA.
- Must be willing to travel to assigned locations and drive street health medicine van to and from various service sites as needed.
