About the role

  • Directs and coordinates all patient care services provided by the agency. Ensures that initial, comprehensive, and ongoing patient assessments are performed timely and according to regulatory standards. Reviews and approves individualized plans of care, ensuring orders are complete, appropriate, and followed. Coordinates and integrates all services provided to each patient. Ensures appropriate reassessment and revision of care plans in response to changes in condition. Provides direct supervision of clinical staff, including registered nurses. Conducts competency evaluations, skills assessments, and performance reviews of clinical staff in accordance with agency policy and CDPH regulations. Ensures that all personnel are appropriately qualified, trained, and supervised for the care and services they provide. Ensures compliance with Title 22, and all CDPH licensing requirements. Maintains knowledge of, and implements, CDPH All Facility Letters (AFLs) and other directives. Participates in quality assurance and performance improvement (QAPI) activities as required by state regulations Provides direct nursing services within scope of RN practice when necessary. Performs supervisory visits to evaluate the quality of care and patient satisfaction. Maintains accurate and complete clinical records. Assists with survey readiness and participates in state inspections and audits. Collaborates with the Administrator in policy development, staff orientation, in-service education, and compliance monitoring. Performs additional responsibilities as assigned by the Administrator in alignment with agency needs and regulations.

Requirements

  • The Alternate Director of Patient Care Services shall qualify for the position by fulfilling the requirements under one of the following categories, unless the individual has been previously approved for such employment by a program flexibility issued for the individual's current position at the home health agency prior to April 1, 1995: (1) A registered nurse with a baccalaureate or higher degree in nursing or other health related field with three years of experience within the last five years in a home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity; or (2) A registered nurse with four years experience within the last five years in a home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity. The Alternate Director of Patient Care Services shall have sufficient background knowledge and expertise in clinical decision-making for the patient population of the home health agency to meet the needs of patients and to contribute to Quality Management review and evaluation . Essential Duties and Responsibilities: The Alternate DPCS, when acting on behalf of the DPCS, shall have the authority and responsibility for: Directs and coordinates all patient care services provided by the agency.
