Assist the Bereavement Program Coordinator with ensuring bereavement services are provided to the families/caregivers of hospice patients in accordance with the interdisciplinary plan of care.
Provide bereavement counseling to the individual's family after the individual's death for a period up to one year following the patient's death.
Ensure bereavement services reflect the needs of the bereaved.
Coordinate with the interdisciplinary team and community resources to support bereaved persons.
Document and maintain records per Medicare, accrediting bodies, and state regulations applicable to bereavement programs.
Respond appropriately to sensitive issues and provide emotional support to families and caregivers.
Work with minimal supervision, prioritize caseloads, and manage time-pressure and changing priorities.
Demonstrate professionalism, participate in orientation and annual core competency assessments.
Requirements
Master's: In social work or related field
Equivalent experience will be accepted in lieu of the required degree or diploma.
DL-Valid Drivers License
Automobile Insurance
BLS- Basic Life Support Certification
2 years recent relevant experience
Understanding of psychosocial implications of illness, hospice and/or home care death and dying issues and dynamics of grief.
Knowledge of Medicare, Accrediting bodies, State and all other regulations applicable to Bereavement programs
Knowledge of community resources available to bereaved persons.
Excellent computer skills.
Must be able to discern sensitive issues and respond appropriately.
Must be sensitive to the human needs and situations encountered in the provision of hospice and bereavement care.
Must be detail-oriented and creative.
Must have excellent organizational skills, interpersonal skills, problem solving abilities, ability to prioritize work.
Must be able to work effectively with a minimum of supervision.
Must be able to work under time-pressure and changing priorities and be able to deal with potentially conflicting demands.
Demonstrate professionalism when interacting with clients and co-workers.
Must be able to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts; effectively plan and organize and prioritize work, and to effectively solve unique problems as they arise or identify when to consult supervisor.
Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements.
At orientation and annually thereafter, must be able to meet core competencies for position and area of specialty, as appropriate.
