Manager

• Collaborate with clients to evaluate safety and social needs, housing history and barriers, financial management and budgeting, and develop and achieve individualized goals
• Maintain a caseload of between 12-18 clients at a time, providing client-centered, trauma-informed services
• Provide mobile advocacy services by regularly meeting with clients in a safe location in the community
• Engage with clients to create individualized safety plans and educate clients on the dynamics of domestic violence

• Coordinate the provision of monthly rental assistance and track all payments provided
• Document all communication with clients in detailed case notes; ensure client files and data are regularly updated
• Ensure services are implemented in accordance with agency philosophy, best practices, grant requirements, and all applicable federal, state, and local regulations
• Partner with community providers and allied professionals to ensure the delivery of comprehensive, wraparound services
• Participate in regular trainings, staff meetings, and one-on-one supervision meetings with the Senior Case Manager
• Proficiently utilize Microsoft Office software as a part of day-to-day work requirements

Rapid Re-Housing Case Manager

Events

• Supervise Case Manager(s) providing mobile advocacy to victims of domestic violence, sexual violence and/or human trafficking living in the community
• Provide coaching and support to case management team through regularly scheduled one on one meetings and evaluations
• Support Case Managers in delivering complex case management for cases that demand higher level solutions
• Collaborate with the Director and Assistant Director of DVSH to ensure consistent service delivery that is in line with grant and contract requirements, company policy and procedure, and best practices
• Complete regular reviews of client files and VADATA reports, to ensure accuracy, compliance with the Violence Against Women Act, and adherence to agency standards and best practices
• With support from the Assistant Director, ensure compliance with all grant requirements
• Disseminate information related to tenant rights and responsibilities, housing discrimination, communicating with landlords, basic unit maintenance, lease compliance, and housing search and location techniques
• Develop new partnerships and identify community resources for unmet client needs
• In collaboration with the Assistant Director, implement and oversee new program initiatives
• Verify and approve timesheets and leave requests
• Perform in-depth assessments of clients’ physical, mental health and social needs, life skills competency, employability, housing history and barriers, educational needs, financial management skills, strengths and needs, and other necessary areas, in conjunction with the client, to develop and achieve housing goals.
• Administer flexible financial assistance to support client goals related to safety and stability
• Provide mobile advocacy services by meeting with assigned clients regularly in a safe location in the community
• Provide crisis intervention and developing individualized safety plans as needed; assisting victims of violence in filing for protective orders and completing affidavits; and accompanying clients to court proceedings and various appointments (i.e. attorney meetings, protective order hearings, custody hearings, and housing interviews)
• Provide housing search assistance to clients by identifying housing leads, assisting clients to apply for units, and accompanying clients on housing tours
• Establish relationships with local landlords to negotiate lease terms and advocate on behalf of clients
• Collaborate with clients to develop monthly budgets; providing money management education and connections to financial literacy programs as needed
• Document all contacts with clients in client file and VADATA as required; maintaining complete client records in accordance with agency and best practice standards
• Provide domestic violence psychoeducation to clients utilizing tools such as The Power and Control Wheel and the Cycle of Violence
• Empower clients and their household members as they work toward their individual goals; completing ongoing reviews of client service plans
• Collaborate with community providers and allied professionals to ensure the delivery of comprehensive wrap-around services
• Participate in staff meetings and training as directed by supervisor
• Function as a contributing member of a multidisciplinary team with Shelter House Inc. employees, deployed staff, and involved agencies

Senior Prevention Case Manager

• Proactively identify and secure housing opportunities across Fairfax County and surrounding areas
• Build and maintain relationships with landlords willing to work with clients facing housing barriers
• Maintain and regularly update a landlord database with relevant contact and leasing information
• Develop and distribute marketing materials to recruit and retain landlord partners
• Maintain ongoing communication with landlords to support positive, long-term partnerships
• Conduct initial housing planning meetings and complete housing assessments within two weeks of program entry
• Collaborate with case managers to support clients in developing and following individualized housing plans and budgets
• Assist clients with housing searches, including reviewing housing logs and identifying appropriate units
• Accompany clients to housing viewings and assist with rental applications
• Facilitate communication between clients and landlords throughout the lease-up process
• Support lease negotiations in collaboration with clients and case managers
• Assist clients with accessing rental assistance and other housing resources
• Develop and facilitate trainings on tenant rights and responsibilities, lease agreements, and housing stability
• Provide education on available housing programs, including Fair Market Rate units, voucher programs, transitional housing, and permanent supportive housing
• Support clients during the move-in process and coordinate with internal and external partners as needed
• Maintain accurate and timely documentation in HMIS and other required systems
• Collaborate with local housing partners, including the Office to Prevent and End Homelessness, nonprofits, and community organizations
• Participate in staff meetings, trainings, and team initiatives
• Ensure compliance with all Shelter House policies and program requirements
• Utilize agency systems and tools to support daily operations
• Safely operate Shelter House vehicles when transporting clients
• Demonstrate Shelter House core values in all interactions

Housing Locator

• Ensure each participant feels welcome in the shelter, has basic needs items and supplies throughout their stay, and understands community guidelines and safety procedures
• Provide relational and responsive case management support to each household
• With each household, develop Housing First focused service plans which are individualized to the household’s unique housing need; assist each household through the housing location process.
• Complete any relevant assessments or referrals needed to support the participant’s housing plan and needs
• Empathetically and firmly address any safety issues that may arise during the participant’s stay in shelter and work to help refocus efforts on their housing plan
• Once housing is secured, partner with participants to outline what supports are needed to address housing barriers and needs, and to develop an agreed upon plan to achieve housing related goals and to maintain housing stability
• Assist participants in securing needed or requested basic needs items and services such as clothing, employment opportunities, benefits, health resources, employment, childcare, educational resources, etc.
• Work to collect financial information from participants as well as formulate a working budget with them
• In collaboration with the Housing Locator, communicate and establish working relationships with local landlords to negotiate lease terms and advocate on behalf of participants
• Assist with developing a rental assistance package for participants utilizing a variety of funding sources, properly document the distribution of funds.
• Assist participants in accessing financial resources, including rapid re-housing rental assistance, and prepare and submit necessary documentation
• Function as a contributing member of a multidisciplinary team with Shelter House Inc. employees, deployed staff, and involved agencies to ensure participants receive needed services in a timely manner
• Assure that all relevant information pertaining to participants is disseminated to all appropriate staff members in a timely manner
• Encourage and support the household to focus their efforts on attaining the objectives specified in their Service Plan
• Assist participants in planning for discharge from the program
• Document contact with all participants in a timely manner (within 48 hours) using internal tracking methods, participant files and HMIS as required
• Develop and facilitate training and workshops for participants on topics including but not limited to budgeting, self-care, seeking employment, mental health, etc.
• Maintain complete participant records in accordance with agency and best practice standards
• Maintain current knowledge and ability to implement emergency procedures including responsibilities as a mandated reporter
• Be on call as directed by supervisor
• Participate in staff meetings and trainings as directed by supervisor
• Commit to being a continuous learner; study best practices implemented by Shelter House and Fairfax County and collaborate with program management to implement
• Empower and support participants in their successful completion of their identified goals
• Safely utilize Shelter House owned vehicles to drive participants to various locations
• Ensure compliance with all agency policies
• Proficiently operate a personal computer, fax, printer, copier and Microsoft Office products (email, calendar, drive, etc).

Shelter Case Manager

Community Manager

• The Housing Locator is responsible for cultivating and maintaining positive relationships with both landlords and clients for housing placement supports.
• The Housing Locator facilitates communication between landlords and clients as needed and empowers clients by providing housing related training (ex: maintaining a household, tenant rights, negotiating leases, landlord communication, etc.).
• The Housing Locator works collaboratively with the case management team to assist households in formulating and following a housing plan and a budget and identifying housing opportunities.
• Proactively and creatively seek new housing opportunities and resources in Fairfax and other localities to assist household with locating housing.
• Build and maintain relationships with area landlords who will work with households with multiple barriers.
• Maintain a Landlord Database to include contact information for all landlords that have signed leases with clients served.
• Regularly communicate with landlords to maintain a positive, working relationship wherever possible.
• Develop and utilize marketing materials geared toward landlord and partner recruitment.
• Conduct initial Housing planning meetings with each household entering the RRH & Prevention Services within first two weeks: Completing housing assessments to identify budget & income.
• Regularly collaborate and communicate with Case Managers regarding clients’ housing updates.
• Provide and review housing search logs with clients on weekly basis for guidance and support.
• Facilitate initial communication between landlords and the clients to assist with lease-up process.
• Accompany clients to view potential housing options and assist with filling out application process.
• Negotiate leases on behalf of clients in collaboration with case managers and the client.
• Maintain case documentation using HMIS and in accordance with agency and best standard practice.
• Assist households with obtaining rental assistance money utilizing a variety of funding sources.
• Disseminate relevant and available housing options information to clients. This includes but is not limited to available housing options at or below Fair Market Rate rentals (as established by HUD), voucher programs, public housing programs, transitional housing programs, PSH, etc.
• Assist households as needed during their move into permanent housing in collaboration with other involved staff and partners.
• Develop and facilitate trainings and workshops for clients on topics including but not limited to tenant rights and responsibilities, housing discrimination, communicating with landlords, how to read a lease, identifying roommates, maintaining a household, etc.
• Collaborate with other Housing Locators within Fairfax County to stay current on best practices and local resources.
• Participate in staff meetings and trainings as directed by supervisor.
• Ensure compliance with all agency and program policies.
• Proficiently utilize Google Suites applications (email, calendar, drive, docs, etc) as a part of day to day work requirements.
• Safely utilize Shelter House owned vehicles to drive clients to various locations.
• Demonstrate positive role modeling for clients and contribute to the health, well-being and comfort of clients by exhibiting the Shelter House Core Values; Inclusivity, Collaboration, Accountability, Respect and Empowerment.
