• **How you will contribute: **
• - Provide crisis intervention and develop an individualized safety plan
• - Assist victims in filing for protective orders and completing affidavits
• - Accompany clients to court proceedings and various appointments (i.e., attorney meetings, protective order hearings, custody hearings, and housing interviews)
• - Perform assessment of client’s physical, mental health and social needs, employability, educational needs, quality of life issues, strengths and needs, and other necessary areas
• - Coordinate services amongst mental health service workers, service providers, advocates, the legal system and other involved parties
• - Coordinate children’s services including education and daycare, programming and counseling
• - Meet with assigned clients regularly and provide intensive case management and advocacy services

• - Empower clients to meet goals developed in their individual service plan and collaborate with client to develop a plan for exiting shelter
• - Collaborate with Resource Coordinator’s to ensure comprehensive client driven service delivery.
• - Advocate for clients and complete referrals to meet client needs.
• - Ensure that Artemis House staff are aware of clients’ needs by completing daily documentation and communicating client updates to all staff
• - Assist with room turnover, client moves, and preparation for clients entering and exiting the program.
• - Listen effectively, delivers effective written and oral communication.
• - Maintain familiarity with agency and program emergency procedures
• - Participate in staff meetings and trainings as directed by supervisor
• - Function as a contributing member of a multidisciplinary team with Shelter House Inc. employees and involved agencies to assist client with their identified goals
• - Ensure compliance with all agency policies
• - Proficiently operate a personal computer, fax, printer, copier and Microsoft Office products (email, calendar, drive, etc.).

Case Manager

Events

• **Responsibilities:**
• - Provide leadership and direction to the Domestic Violence Homelessness Prevention Program through the following activities:
• - Supervise Case Manager(s) providing mobile advocacy to victims of domestic violence, sexual violence and/or human trafficking living in the community
• - Provide coaching and support to case management team through regularly scheduled one on one meetings and evaluations
• - Support Case Managers in delivering complex case management for cases that demand higher level solutions
• - Collaborate with the Director and Assistant Director of DVSH to ensure consistent service delivery that is in line with grant and contract requirements, company policy and procedure, and best practices
• - Complete regular reviews of client files and VADATA reports, to ensure accuracy, compliance with the Violence Against Women Act, and adherence to agency standards and best practices
• - With support from the Assistant Director, ensure compliance with all grant requirements
• - Disseminate information to related to tenant rights and responsibilities, housing discrimination, communicating with landlords, basic unit maintenance, lease compliance, and housing search and location techniques
• - Develop new partnerships and identify community resources for unmet client needs
• - In collaboration with the Assistant Director, implement and oversee new program initiatives
• - Verify and approve timesheets and leave requests
• Provide case management services to a reduced caseload by:
• - Performing in-depth assessments of clients’ physical, mental health and social needs, life skills competency, employability, housing history and barriers, educational needs, financial management skills, strengths and needs, and other necessary areas, in conjunction with the client, to develop and achieve housing goals.
• - Administering flexible financial assistance to support client goals related to safety and stability
• - Providing mobile advocacy services by meeting with assigned clients regularly in a safe location in the community

• - Documenting all contacts with clients in client file and VADATA as required; maintaining complete client records in accordance with agency and best practice standards
• - Providing domestic violence psychoeducation to clients utilizing tools such as The Power and Control Wheel and the Cycle of Violence
• - Empowering clients and their household members as they work toward their individual goals; completing ongoing reviews of client service plans
• - Collaborating with community providers and allied professionals to ensure the delivery of comprehensive wrap-around services

Senior Prevention Case Manager

Manager

• Work with a caseload of 10-20 single and family households in supportive housing
• Support the housing first model using a trauma informed care approach to maximize client success and long-term self-sufficiency
• Conduct individual needs assessments and use outcomes to determine supports needed to maintain stability and achieve goals
• Develop Individualized Service Plans and assist clients in planning for discharge/graduation
• Assist clients in securing appropriate services and coordinate delivery of services (employment, housing, finance, mental health, disability resources, etc.)
• Provide support to clients experiencing crises and implement emergency procedures
• Create and maintain financial budgets and money management education with clients; ensure rent and utilities are paid timely
• Maintain complete client records in accordance with agency standards and timely documentation in client file and HMIS
• Participate in on-call rotation, staff meetings, and trainings as directed by supervisor
• Function as a contributing member of a multidisciplinary team with Shelter House Inc. and partner agencies
• Safely utilize Shelter House owned vehicles to drive clients to various locations

Supportive Housing Case Manager

• Residential Coordinator provides 24-hour awake coverage and supervision of the domestic violence shelter; lead person on shift acting as first point of contact for client needs and support; crisis intervention; facilities management; ensuring safe and efficient operation of front desk and positive living environment; ensure safety of shelter residents and property; monitor calls and inquiries maintaining confidentiality; answer crisis calls from Fairfax County Domestic Violence Hotline and complete safety plans and needs assessments; update daily log by documenting interactions and significant events; work closely with case managers to support service plans and maintain communication regarding client progress; accompany clients to court and appointments; complete shift exchange with incoming/outgoing staff; ensure documentation compliance; evaluate and provide for basic needs of shelter clients; maintain dignity and confidentiality; administer emergency procedures; conduct assessments and intakes for clients in imminent danger; assist with medication delivery and security; participate in staff meetings and trainings; ensure compliance with agency policies; serve as emergency personnel and report to work regardless of circumstances; exemplify core values; other duties as assigned

Residential Coordinator

Program Manager

• Provide leadership, oversight, and support for Rapid Re-Housing and Permanent Supportive Housing programs in Loudoun County
• Supervise a team of Case Managers and provide coaching and development support
• Support Case Managers in delivering complex case management and facilitate client appeals process
• Assist in development of termination plans and attend/facilitate client meetings
• Ensure services align with trauma-informed care, Housing First, and harm-reduction best practices
• Review documentation, data entry, timesheets, and conduct quarterly file audits
• Ensure efficient program operations, manage advertisement of openings, referral tracking, client lists, workflows
• Oversee program budgets, track spending, and ensure appropriate use of funds
• Complete or delegate assessments, approve requests for financial assistance per agency/state requirements
• Maintain communication with property management/landlords and conduct regular unit inspections per HUD standards
• Calculate client rental payments, track receipts, issue pay or quit notices per policy
• Hire vendors or report maintenance issues, maintain housing unit records, and fulfill on-call emergency duties
• Participate in inter-professional collaborative meetings, staff meetings, and training

Community Housing Program Manager

• Lead the shelter case management team to provide comprehensive services to households experiencing homelessness
• Carry a caseload at the off-site hotel location and provide direct case management
• Develop and implement individualized housing-first service plans to secure and maintain permanent housing
• Facilitate housing-related tasks including collecting documents and preparing rental assistance packages
• Assist with developing rental assistance packages, document fund distribution, and lead recertification processes
• Collaborate with community partners, service providers, hotel staff, LHSC site, and Assistant Director of Programs
• Supervise, support, and provide guidance to Case Managers, including phone screenings and diversion support
• Ensure accurate and timely HMIS data entry, monthly data clean-ups, and oversee direct reports' data entry
• De-escalate safety concerns and complete incident documentation before shift end
• Participate in intakes, on-call manager rotations, staff meetings, trainings, and file audits
• Manage tasks and scanning to maintain complete and current participant charts
• Facilitate ongoing reviews of service plans and lead meetings and supervision sessions as needed

Senior Case Manager

• The Case Manager will work with 10-20 families in our Shelter, Prevention, and Rapid Rehousing Programs. Work based both in the community as well as at the Fair Ridge Family Shelter. Support the housing first model with a trauma informed care approach to maximize client success and long-term self-sufficiency; assist families with obtaining and maintaining housing.
• Conduct individual needs assessments, program planning, empowering clients’ progress toward housing, and assisting families in locating or maintaining housing options.
• Link families with services and resources after housing; assist with budgeting, tenant skills, income increases, and community service linkages.
• Serve as the primary point of contact for assigned client services and provide guidance and support toward self-sufficiency.
• Form relationships with landlords and community partners to secure housing; proactively seek housing opportunities in Fairfax and other localities.
• Plan and conduct Family Shelter Planning Team meetings; assist clients with developing housing plans and maintaining stable housing.
• Assist clients in securing appropriate services (employment, housing, finance) and improve money management by developing budgets.
• Assist with rental assistance packages, document funds distribution, lead recertification process if needed.
• Maintain client records, timely documentation in HMIS, participate in Intake Phone rotation, staff meetings and trainings, and safely utilize Shelter House vehicles.
