Manager

• Collaborate with clients to evaluate safety and social needs, housing history and barriers, financial management and budgeting, and develop and achieve individualized goals

Rapid Re-Housing Case Manager

• Provide relational and responsive case management support to each family and empower and support clients in their successful completion of their identified goals
• Review referrals and speak with families to determine program eligibility and assess household needs.
• With each assigned family, develop Housing First focused service plans which are individualized to the household’s unique housing need; assist each household through the housing location process
• Complete any relevant assessments or referrals needed to support the client’s housing
• Once housing is secured, partner with families to outline what supports are needed to address housing barriers and needs, and to develop an agreed upon plan to achieve housing related goals and to maintain housing stability
• Assist family in securing needed or requested basic needs items and services such as clothing, employment opportunities, benefits, health resources, employment, childcare, educational resources, etc.
• Work to collect financial information from families as well as formulate a working budget with them
• Acting as Housing Locator, communicate and establish working relationships with local landlords to negotiate lease terms and advocate on behalf of clients
• Assist Shelter Case Managers in housing location for their clients
• Assist with developing a rental assistance package for all clients utilizing a variety of funding sources, properly document the distribution of funds, and lead the recertification process if families require additional funds to maintain housing.
• Function as a contributing member of a multidisciplinary team with Shelter House Inc. employees, deployed staff, and involved agencies to ensure clients receive needed services in a timely manner
• Assure that all relevant information pertaining to families is disseminated to all appropriate staff members in a timely manner
• Encourage and support the families to focus their efforts on attaining the objectives specified in their Service Plan
• Assist clients in planning for discharge from the program
• Document contact with all families in a timely manner (within 24 hours) using internal tracking methods, participants files and HMIS as required

Director

• Oversee daily operations of a large emergency shelter for households with minor children
• Collaborate with Fairfax County’s Office to Prevent and End Homelessness

Senior Rapid Rehousing Case Manager

Case Manager

• - Provide leadership and direction to the Domestic Violence Homelessness Prevention Program through the following activities:
• - Supervise Case Manager(s) providing mobile advocacy to victims of domestic violence, sexual violence and/or human trafficking living in the community
• - Provide coaching and support to case management team through regularly scheduled one on one meetings and evaluations
• - Support Case Managers in delivering complex case management for cases that demand higher level solutions

Supportive Housing Case Manager

• Residential Coordinator provides 24-hour awake coverage and supervision of the domestic violence shelter; lead person on shift acting as first point of contact for client needs and support; crisis intervention; facilities management; ensuring safe and efficient operation of front desk and positive living environment; ensure safety of shelter residents and property; monitor calls and inquiries maintaining confidentiality; answer crisis calls from Fairfax County Domestic Violence Hotline and complete safety plans and needs assessments; update daily log by documenting interactions and significant events; work closely with case managers to support service plans and maintain communication regarding client progress; accompany clients to court and appointments; complete shift exchange with incoming/outgoing staff; ensure documentation compliance; evaluate and provide for basic needs of shelter clients; maintain dignity and confidentiality; administer emergency procedures; conduct assessments and intakes for clients in imminent danger; assist with medication delivery and security; participate in staff meetings and trainings; ensure compliance with agency policies; serve as emergency personnel and report to work regardless of circumstances; exemplify core values; other duties as assigned

Residential Coordinator

Program Manager

• Provide leadership, oversight, and support for Rapid Re-Housing and Permanent Supportive Housing programs in Loudoun County
• Supervise a team of Case Managers and provide coaching and development support
• Support Case Managers in delivering complex case management and facilitate client appeals process
• Assist in development of termination plans and attend/facilitate client meetings
• Ensure services align with trauma-informed care, Housing First, and harm-reduction best practices
• Review documentation, data entry, timesheets, and conduct quarterly file audits
• Ensure efficient program operations, manage advertisement of openings, referral tracking, client lists, workflows

Community Housing Program Manager

• Lead the shelter case management team to provide comprehensive services to households experiencing homelessness
• Carry a caseload at the off-site hotel location and provide direct case management
• Develop and implement individualized housing-first service plans to secure and maintain permanent housing
• Facilitate housing-related tasks including collecting documents and preparing rental assistance packages
• Assist with developing rental assistance packages, document fund distribution, and lead recertification processes
• Collaborate with community partners, service providers, hotel staff, LHSC site, and Assistant Director of Programs
• Supervise, support, and provide guidance to Case Managers, including phone screenings and diversion support
• Ensure accurate and timely HMIS data entry, monthly data clean-ups, and oversee direct reports' data entry
• De-escalate safety concerns and complete incident documentation before shift end
• Participate in intakes, on-call manager rotations, staff meetings, trainings, and file audits

Senior Case Manager

• The Case Manager will work with 10-20 families in our Shelter, Prevention, and Rapid Rehousing Programs. Work based both in the community as well as at the Fair Ridge Family Shelter. Support the housing first model with a trauma informed care approach to maximize client success and long-term self-sufficiency; assist families with obtaining and maintaining housing.
• Conduct individual needs assessments, program planning, empowering clients’ progress toward housing, and assisting families in locating or maintaining housing options.
• Link families with services and resources after housing; assist with budgeting, tenant skills, income increases, and community service linkages.
• Serve as the primary point of contact for assigned client services and provide guidance and support toward self-sufficiency.
• Form relationships with landlords and community partners to secure housing; proactively seek housing opportunities in Fairfax and other localities.
• Plan and conduct Family Shelter Planning Team meetings; assist clients with developing housing plans and maintaining stable housing.
• Assist clients in securing appropriate services (employment, housing, finance) and improve money management by developing budgets.
