• RADIAS Health is hiring a full-time Waivered Services Case Manager to join the Waivered Services Case Management team;The Waivered Services Case Manager assists clients on a waiver to access needed waiver and State plan services, as well as needed medical, social, educational and other services, regardless of the funding source;Supports clients deemed at risk of placement in a Nursing Facility, Neurobehavioral Hospital or ICF/DD;Sets up and coordinates services and medical equipment necessary for least restrictive setting;Provides ongoing monitoring of services and serves as an advocate when assistance is needed;Goal development and service planning utilizes Person-Centered principles;Assist clients on the CADI, BI, EW and DD waivers in Hennepin County to access needed waiver and state plan services;Develop and implement a plan of care (CSSP) based on MNChoices assessment results and within county approved allocation;Inform clients and/or legal guardians of service options and assist in identifying providers and accessing services;Plan and arrange services and inform county waiver contact for MMIS service agreement entry;Provide services at client's residence or other community locations as specified in the CSSP;Provide crisis intervention services including referrals for emergency needs;Respond to Vulnerable Adult reports in a timely manner and document responses;Determine client needs for residential treatment and select appropriate placement;Monitor client progress towards established goals and determine termination/continuation;Consult with departmental staff, other county departments and community service professionals;Maintain accurate case records and complete forms for documentation;Track and provide data used to monitor trends and developments;Track billable time worked, by client, for reimbursement
