Communicate and collaborate closely with physicians and other members of the care team to provide consistent quality care for patients.
Accept responsibility and accountability for assigned patients as a clinic nurse and care manager to plan, implement and evaluate care.
Assist patients/families, staff and systems to achieve high quality, evidence based, cost-effective, and patient focused outcomes.
Work within a collaborative care team through development of all aspects of clinical workflow and care management operations within the medical home model.
Develop clinical workflow and care management operations and participate in medical home model activities.
Requirements
Graduate of an accredited school of nursing.
Associate’s Degree in Nursing
Montana Registered Nurse License upon hire.
3 years of Professional Nursing Experience
1 year of Care management or similar experience
Preferred: Bachelor’s Degree in Nursing
