- Lead the delivery of a clinical and business model of the Senior Care Options Transitions/Post-Acute programs
- Oversee day-to-day operations of nurse care managers
- Manage and implement targeted interventions for cost/utilization drivers
- Collaborate with interdisciplinary team to develop geriatric-focused care management program
- Accountable for recruitment, evaluation, management, and development of clinicians overseen
Requirements
- Experience in care management and clinical operations
- Ability to oversee day-to-day operations of nurse care managers
- Knowledge of care coordination and population health
- Understanding of data points related to utilization and cost
- Excellent collaboration skills with interdisciplinary teams
- Ability to monitor Utilization metrics and manage clinical performance
- Subject Matter Expertise (SME) related to SCO program regulatory requirements
