• Provides professional occupational therapy services to OMC customers
• Acts in a manner consistent with the State’s professional practice act for occupational therapy and OMC’s policies and procedures
• Organizes and conducts medically prescribed programs to restore function, prevent disability and help patients reach their maximum performance
• Demonstrates knowledge of the principles of growth and development over the life span and assesses data reflective of the patient’s status
• Interprets patient data to develop a treatment plan and provide care needed

Occupational Therapist, PT

Manager

• Assesses, plans, implements, coordinates and evaluates the nursing care of patients in their place of residence
• Promotes continuity of care for patients on their assigned caseload
• Provides direct patient care and supervises the care provided by LPNs and HHAs
• Works in collaboration with all home health disciplines to provide care for patients
• Applies principles of learning in teaching patient’s self-management and methods to regain independence
