Chaplain

• Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner
• Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs
• Serves as liaison and support to community chaplains and spiritual counselors
• Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision
• Documents direct services and ongoing communication with community chaplains and spiritual counselors
• Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group

Field Chaplain

Counselor

• Providing outreach to hospice families to determine their grief support needs.
• Completing a bereavement assessment of the hospice family’s needs and developing an individualized plan of care based on the bereavement assessment.
• Providing grief counseling to the bereaved on a time-limited basis as deemed helpful and appropriate.
• Referring to the community those bereaved who need mental health or other community resources when needed.
• Maintaining clinical records related to grief support services performed.
• Promoting grief education and support to the hospice team on an individual or group basis.
• Participating in the Interdisciplinary Team Meetings to determine grief support needs of hospice families.

Grief Counselor

General

• Conduct a needs assessment for hospital education with a focus on early hospice admissions
• Implement hospital education presentations/conversations
• Establish and maintain physician relationships to update, educate and support as needed
• Create activity tracer documentation which includes who, when, where participants and next steps
• Assess, manage and seek approval for GIP level of care
• Support patient and family during discharge process

Clinical Liaison

• Enhance communication to ensure existing team members receive support in the field
• Conduct weekly IDT meetings for the Pediatric team
• Guide new employees with orientation and education in hospice care
• Perform timely performance check-ins with their team on a quarterly basis
• Participate in ride-a-long visits with their team on a quarterly basis and as needed
• Maintain a small caseload
• Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs

Pediatric Team Leader

• Oversee a team of RN Case Managers, Social Workers, and Chaplains
• Enhance communication to ensure existing RN Case Managers, Social Workers, and Chaplains receive support in the field
• Conduct weekly IDT meetings for their designated team
• Guide new staff with new employee orientation and education in hospice care
• Perform timely performance check-ins with their team on a quarterly basis
• Maintain a small caseload
• Complete an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs
• Participate in on call coverage

Hospice Team Lead

• Assist with menu planning
• Attend quarterly dietary in-services
• Review nutritional assessments & make recommendations for patients
• Could see patients in In-Patient Unit or patients' private homes
• Attend weekly Pediatric interdisciplinary group meetings
