About the role

Key responsibilities & impact

Enhance communication to ensure existing team members receive support in the field
Conduct weekly IDT meetings for the Pediatric team
Guide new employees with orientation and education in hospice care
Perform timely performance check-ins with their team on a quarterly basis
Participate in ride-a-long visits with their team on a quarterly basis and as needed
Maintain a small caseload
Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs

Requirements

What you’ll need

RN with minimum of 3 years of Hospice experience
Prior supervisory experience
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements; and is in good working order
