About the role
Key responsibilities & impact- Providing outreach to hospice families to determine their grief support needs.
- Completing a bereavement assessment of the hospice family’s needs and developing an individualized plan of care based on the bereavement assessment.
- Providing grief counseling to the bereaved on a time-limited basis as deemed helpful and appropriate.
- Referring to the community those bereaved who need mental health or other community resources when needed.
- Maintaining clinical records related to grief support services performed.
- Promoting grief education and support to the hospice team on an individual or group basis.
- Participating in the Interdisciplinary Team Meetings to determine grief support needs of hospice families.
Requirements
What you’ll need- Master’s degree in social work from an accredited graduate program.
- Minimum of one year of experience as an LPC or LSW (preferred), healthcare or hospice experience preferred.
- Bilingual proficiency in English and Spanish is strongly preferred.
- Experience counseling children and teens is highly preferred.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements; and is in good working order.
