About the role
Key responsibilities & impact- Conduct a needs assessment for hospital education with a focus on early hospice admissions
- Implement hospital education presentations/conversations
- Establish and maintain physician relationships to update, educate and support as needed
- Create activity tracer documentation which includes who, when, where participants and next steps
- Assess, manage and seek approval for GIP level of care
- Support patient and family during discharge process
Requirements
What you’ll need- Minimum of 2 years hospice and palliative care experience
- Valid license as a registered nurse in the State of Illinois
- RN required; BSN preferred
- CHPN preferred
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements; and is in good working order
