Key responsibilities & impact

Conduct a needs assessment for hospital education with a focus on early hospice admissions
Implement hospital education presentations/conversations
Establish and maintain physician relationships to update, educate and support as needed
Create activity tracer documentation which includes who, when, where participants and next steps
Assess, manage and seek approval for GIP level of care
Support patient and family during discharge process

Requirements

What you’ll need

Minimum of 2 years hospice and palliative care experience
Valid license as a registered nurse in the State of Illinois
RN required; BSN preferred
CHPN preferred
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements; and is in good working order
