• Conduct thorough and complete chart reviews for PACE applicants, evaluating medical records, diagnostic history, functional status, and clinical documentation to capture all factors relevant to eligibility determination and potential H&S denial
• Present cases to Site Medical Directors as needed based on departmental guidelines and policy, ensuring all clinical findings are clearly communicated to support informed eligibility decisions
• Collaborate closely with Enrollment RNs (ERNs) and Site Medical Directors to ensure documentation gaps are identified and resolved prior to submission; serve as the escalation point when ERNs are unable to obtain required diagnostic information
• Provide clinical expertise for Health & Safety determinations, including participation in H&S Committee meetings; when chart review reveals diagnostic gaps that cannot be resolved by the ERN, conduct virtual clinical assessments to obtain or confirm required diagnoses and ensure chart completeness prior to submission
• Review ILOC clinical summaries to ensure clear connections between medical conditions, functional limitations, and PACE services are documented to regulatory standard
• Support quality improvement initiatives aimed at maintaining high first-pass DHCS approval rates and reducing documentation deficiencies
• Serve as a clinical resource for the Pre-Enrollment team on complex cases, supporting accuracy and thoroughness in ILOC documentation to protect the organization's H&S denial integrity

Central Pre-Enrollment Nurse Practitioner

Associate

• Organize, maintain, and update participant medical records in the EHR system, including creating and indexing documentation from electronic sources
• Process, manage, retrieve, and follow up on medical records requests with external providers in accordance with HIPPA and organizational policies, ensuring timely receipt
• Coordinate release of information via electronic transmission, fax, mail, or secure delivery methods

Analyst

• Supports quality improvement infrastructure across PACE centers
• Maintains core quality data systems

Quality Improvement Analyst

Manager

• Support implementation of a Contract Management System, including developing and refining workflows that will serve to streamline our contracting processes from intake through review, approval, execution, and inventorying
• Manage contract review and Performance. Assist with processing standard form agreements such as NDAs, short-term consulting agreements, and BAAs and ensure all post-execution obligations (e.g. reporting, renewal dates) are tracked.
• Corporate Entity Management. Oversee and manage corporate records of each legal entity, including formation documents and applicable state licenses and qualifications. Monitor upcoming due dates and file periodic reports with Secretary of State offices (or equivalent).
• Support Board Operations. Assist with managing communications to Board members, including transmitting Board materials, securing signatures for Board resolutions, and ensuring accurate records.
• General Paralegal Support. Track and renew and facility licenses and permits and ensure appropriate exclusion screening for vendors and contractors.
• Project Support. Provide project management and administrative support for ongoing projects such state expansion initiatives.
• Collaboration and Communication. Develop and maintain relationships with all levels of management and staff across various departments, particularly forging strong partnerships with the compliance and finance teams.

Contracts Manager

Call Center Representative

• Serve as the central telephonic point of contact for prospective participants, caregivers, and referral partners, building trust-based relationships from the very first conversation.
• Make outbound follow-up calls to prospects who have expressed interest in learning more about Habitat Health PACE, including those from our partner organizations, such as Kaiser Permanente.
• Support inbound campaigns by answering questions, surfacing underlying needs, and educating prospects about PACE services, eligibility, and how Habitat Health can support their care.
• Understand the needs and requirements of prospective participants and their families, and clearly articulate Habitat Health's unique value proposition in the PACE landscape.
• Apply strong phone engagement skills including rapport building, active listening, qualification questioning, addressing objections and concerns, and guiding prospects to a confident next step.
• Assess preliminary PACE eligibility and guide qualifying seniors through early steps of the enrollment process, including support with the Medi-Cal application where appropriate.
• Coordinate the warm handoff to on-the-ground Enrollment Specialists, Outreach Specialists, and Intake Coordinators — ensuring a seamless transition from central touchpoint to local engagement.
• Perform service recovery, address prospect concerns, and de-escalate callers when needed.
• Manage time effectively and prioritize tasks across a high volume of inbound and outbound activity.
• Meet or exceed personal and team targets for call volume, conversion, and warm-handoff goals.
• Maintain accurate record of all prospective participant communication and activity using a Customer Relationship Management (CRM) system and Contact Center system.
• Maintain compliance with all CMS, DHCS, and Habitat Health policies — including TPMO recording obligations and all federal and state marketing requirements.
