Bilingual Social Worker – Cantonese Required

• Contact the client/family/guardian and makes an appointment to meet with them.
• Interview for purposes of obtaining social, emotional, developmental and familial information to form a home based assessment.
• Obtain the completed intake information from client and/or family and conducts a need assessment prioritizing available resources selections.
• Clarify the request for services and the needs of client/family.
• Review and analyze available data; identify barriers and opportunities in clients’ developmental objectives and plans.
• Design, monitor and revise Individual Family Service Plan (IFSP) to meet personal goals and determines client availability within the state mandated timeframes.
• Work with client families to assess needs, preferences and life choices.
• Confirm any immediate needs and provide or arrange for any necessary interventions or services.
• Meets with the applicant/family/guardian within a designated time period and reviews submitted materials.
• Obtain the completed application form from applicant and/or family (or assist in completion of form).
• Provide information regarding Golden Gate Regional Center, Early Start and services available.
• Interview for purposes of obtaining social, emotional, developmental and familial information to form social diagnosis.
• Completes Early Start Assessment report.
• Schedule and coordinate meetings with team and applicants.
• Obtain necessary information to complete Applicant Developmental Evaluation Report (CDER).
• Obtain any additional signed Release of Information forms for additional data base material.
• Submit completed application packet and completed Releases of Information to the Office Assistant – Intake.
• Coordinate assessment team's completion of the CDER at the time of eligibility determination.
• Provides information and referrals for applicants who are not eligible for agency services.
• Research available community resources and services.
• Ensure that services are provided per the Lanterman Act and Title XVII.
• Obtain or collect information or data for use by interdisciplinary team.
• Provide intake eligibility services as needed.
• Provide crises management as needed.
• Review and ensure that the applicants receive appropriate services from existing community resources.
• Empower applicants and their families to fully participate in coordinating and utilizing the resources available.

Bilingual Social Worker – Early Start Assessment, Cantonese Preferred

• Lead the assessment of applicants (0-36 months) applying for services under the Early Start program
• Facilitate IDCT eligibility assessment coordination and develop Initial IFSP within 45 days of referral
• Contact the client/family/guardian to make appointments
• Conduct interviews to obtain social, emotional, developmental and familial information for home-based assessments
• Clarify requests for services and client/family needs
• Design, monitor and revise Individual Family Service Plan (IFSP)
• Work with client families to assess needs, preferences and life choices

Support Engineer

• Serve as first contact for New Hire technical questions and support.
• Respond to and resolve common new hire technology inquiries and access issues.
• Create accounts based on Agency Standards.
• Configure and deploy agency devices such as laptops, mobile phones, office phones for new staff.
• Reset hardware, software and accounts, and prepare returned equipment for new employees.
• Suspend/terminate accounts based on Agency Standard.
• Create and/or update documentation for Agency Standards
• Provides first level of systems support for employees throughout the agency and elicits details of issues in order to properly prioritize and refer issues
• Document all pertinent end user identification information, including name, department, contact information, and nature of problem or issue
• Prioritize, track, and resolve support requests, escalating issues as appropriate. to the appropriately experienced technician
• Record, track, and document the help desk request problem-solving process, including all successful and unsuccessful decisions made, and actions taken, through to final resolution

• Collaborate with business stakeholders to gather and document requirements for SaaS solutions

IT Business Analyst – SaaS Solutions

Director

• Oversees all Social Work activities including Lanterman Services (for ages 3 and older) and the Self-determination Program.
• Serves as a member of the Management Advisory Committee, contributing to agency-wide strategic and operations decision making.
• Coordinates with Executive Director, Department Directors, and other key staff on all aspects of case management for individuals served by the regional center who are 3 years old and older.
• Informs organizational restructuring decisions based on growth in those served, their evolving needs and developments in service options.
• Assesses, reviews, and approves funding requests, ensuring alignment with GGRC policies and Purchase of Service Guidelines.
• Models, practices and teaches Person Centered Thinking, Planning and Action.
• Reviews changes in statutes, regulations, Department of Developmental Services (DDS) Directives and best and promising practices and leads efforts to implement and streamline responses to these changes.
• Coordinates and oversees the on-call coverage for the Regional Center Services.
• Interview, Hire, Train, and Supervise staff
• Provide staff with guidance and feedback on performance
• Hold regular group, department, and individual staff meetings to disseminate information and provide opportunities for feedback on operations
