• Design, coordinate, and implement an overall specialized system of supported living options as well as the development of an evaluation process for follow-up and ongoing monitoring of programs
• Acts as liaison to local providers of supported living services, and serves as communication link between the Agency and the providers.
• Acts as liaison to the Department of Developmental Services for issues regarding supported living services.

• Draft revisions to Agency policies and guidelines and works with DDS to implement revised policies
• Participate in the RFP for potential new housing providers

• Facilitate communications at all levels to solve problems and participate in planning of proactive solutions

• Sets priorities, organizes workload, manages relevant contracts with community partners, and meets agreed upon deadlines.
• Shares related information and articles to staff and stakeholders as appropriate
• Draft written comments on proposed regulations
• Advise others on trends occurring within SLS
• Develops new housing resources
• Negotiate rates
• Recruit, screen, evaluate and contract with vendors
• Establish start-up budgeting and billing procedures
• Presents periodic updates to Board of Directors and Senior Management on housing initiatives.
• Attends Board, Service Providers and providers and meetings with Individuals Supported as needed.
• Coordinates with case management and administration staff regarding authorization of supported living services and service changes.
• Provides training to new employees and consults with staff regarding housing options.
• Contributes to Individual Service Plans for people supported who currently participates in housing services
• Responds to questions related to SLS regulations and interpretation of the regulations
• Prepares and sends required reports related to Supported Living Services within the Agency’s community
• May act as a witness in appeals
• Conducts New Hire and other training relevant to function to staff and community.
• Provides updates on related Performance Contract Measures as requested.

• Serve as liaison to community organization including local government housing departments, non-profit housing development corporations, families/consumer groups interested in residential development

• Maintain all client information as confidential in compliance with the federal Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the California Lanterman Act

Support Living Specialist

• This role offers a hybrid schedule giving you the opportunity to work within home of clients in San Francisco County, our San Francisco office and from the comfort of your own home.
• Advocate for and secure the services individuals with developmental disabilities need to improve their lives.
• Participate in the assessment of individuals supported and their needs and those of their families.
• Develop, implement and coordinate Individual Program Plans and complete documentation as required.
• Manage a wide range of active cases requiring the skills of a professionally trained social worker, including creative planning and acquiring services for individuals supported.
• Conduct ongoing social assessments and overall case planning.
• Design, monitor, and revise the program plan, analyze data, and identify barriers to individual developmental objectives.
• Initiate and terminate regional center funding and ensure that purchased services are provided.
• Prepare required reports and maintain records in compliance with federal and state requirements.

Bilingual Social Worker – Cantonese

• Lead Early Start Applicant Assessment
• Contact clients/families to make appointments
• Conduct interviews for home-based assessments
• Design and monitor Individual Family Service Plans (IFSPs)
• Work with families to assess needs and preferences
• Complete Early Start Assessment reports
• Coordinate assessment meetings and necessary documentation
• Empower applicants and families in resource coordination
• Participate in community meetings and other internal meetings

Bilingual Social Worker – Early Start Assessment, Spanish Required

• Provide enhanced service coordination to persons with developmental disabilities and their families.
• Manage a 1:40 caseload ratio of clients currently served by the Center with low and/or no POS expenditures.
• Utilize a Person-Centered approach to identify barriers in accessing services and supports.
• Enhance relationships between Client/family and Center.
• Empower individuals to partner with Center and other community agencies.
• Assist in the identification of new resources that are culturally relevant to our community.
• Write concise, accurate and objective interdisciplinary notes regarding the current status of the clients and related service coordination activities on a regular basis.
• Enter and maintain TCM/ID notes for each case management activity.
• Attend all meetings with punctuality.
• Conduct meetings, interviews, and evaluations; interpret and document data.
• Visit individuals in their homes, schools and day programs.
• Understand and utilize sound administrative, case management, and counseling principles.
• Represent the agency’s philosophy, policies, and standards in a professional manner in the community and in the office.
• Adhere to a reliable work schedule, i.e. punctuality in reporting to work and in using consistent sign-out methods.
• Record accurately and timely on time sheet and mileage form.
• Engage in Person-Centered Planning meetings with individuals, families, independent facilitators, and others as needed.
• Comply with the use of agency communication technology, e.g. changes voice mail message daily; responds to voice mail and email in a timely manner.
• Maintain current workload with accurate and complete documentation.
• Perform other duties and responsibilities as assigned.
