Community Manager

• Create and execute Purchase of Service contracts with service providers including but not limited to Housing Services, Supported Living Services, and other community services
• Track and authorize invoice payments and funding transfers for Housing Development Organizations and Multifamily Housing invoices and contract activities, ensuring required paperwork is submitted before authorizing payment
• Act as a liaison between GGRC and GGRC legal counsel on contract language and content updates
• Create and implement a contract tracking system to ensure contracts are updated as required
• Create and monitor contracts associated with the Agency’s Request for Proposal funds and other Community Service Unit funds
• Work with staff to develop and implement contracts, including updating contract language, scope of work, milestone goals, and budgets as required for activities associated with Home and Community Based Services Compliance funding and Language Access and Cultural Competency funds
• Track funding expenditures, review invoices, and approve for payment by the agency fiscal department
• Act as a liaison with the Department of Developmental Services (DDS) to secure funding approvals and funding modifications on all Community Services Department project funding
• Track and report progress on contract goals to appropriate GGRC staff
• Review DDS guidance and directives, updating related processes and appropriate staff as needed
• Update GrantVantage and other grant tracking software as applicable to the funding source
• Train applicable staff on related DDS directives, funds and funding use
• Maintain all client information as confidential in compliance with the federal Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the California Lanterman Act.

Community Contracts and Funding Specialist

Bilingual

• The Intake Specialist acts as the first point of contact between potential Early Start and continuing clients and the Regional Center.
• Provide basic information on services and secure information to determine eligibility requirements and potential services.
• Travel to offices and meetings throughout the Bay Area.
• Solicit all intake information from applicants for GGRC services and refers qualified clients to Assessment Team.
• Input intake information and notes into system.
• Confirm client’s “first time” status in Regional Center system.
• Provide general information and referrals regarding resources and services to clients and callers.
• Explain GGRC procedures and policies, including legislation and due process rights.
• Refer to other community resources, when indicated.
• Assure that required timeframes and policies and procedures are met.
• Maintain database of calls and documentation of applicant’s inquiries.
• Completes and maintains all required forms, documentation and reports according to regulations and procedures.
• Determines place of residence in accordance with guidelines.
• Review and determine appropriateness of applications for service.
• Maintain information and referral services.
• Complete Monthly statistics.
• Assist in determining referrals for outreach services.
• Provide administrative support to unit as needed.
• Participate in special projects, and assist with additional duties or tasks, as assigned.
• Maintain all client information as confidential in compliance with HIPAA and the California Lanterman Act.

Intake Specialist, Bilingual – Spanish Required

• Looking for candidates who have a passion to work with individuals with developmental disabilities over the age of 3 years and their families
• Advocate for and secure the services they need to improve their lives
• Participate in the assessment of individuals supported and their needs and those of their families
• Develop, implement and coordinate their Individual Program Plan and complete needed documentation as required
• Manage a wide range of active cases requiring the skills of a professionally trained social worker
• Participate in ongoing social assessments of persons with developmental disability
• Obtain or collect information or data for use by interdisciplinary team
• Design, monitor and revise the program plan, analyze data and identify barriers to individual's developmental objectives
• Initiate and terminate regional center funding and ensure that purchased services are provided
• Prepare required reports and documentation in a timely and accurate manner
• Maintain records in compliance with federal and state requirements
• Act as an advocate for individuals supported and their families to assure receipt of entitled services
• Encourage individuals supported and their families to fully participate in coordinating and utilizing the resources available
• Initiate referrals and monitor progress of vendored services
• Identify community service needs or gaps and assist community resources in the development of needed services
