Key responsibilities & impact

Advocate for and secure services for individuals with developmental disabilities
Participate in assessments of individuals and families
Develop, implement and coordinate Individual Program Plans
Manage a wide range of active cases requiring social work skills
Prepare required reports and documentation in a timely manner
Maintain records in compliance with federal and state requirements
Initiate referrals and monitor progress of services

Requirements

What you’ll need

Knowledge of principles, objective and techniques of counseling individuals
Knowledge of normal and abnormal individual and group behavior, human growth and development of family relationships
Knowledge of community structure and organization
Ability to develop skills allowing for independent work
Excellent communication skills in producing documents in a business manner
Ability to stimulate the cooperation of clients and community agencies
Ability to establish and maintain effective working relationships with others
Bachelor’s Degree from an accredited school of social welfare or related human services field
Fluency in Spanish to perform the duties of the position
Knowledge in the areas of identifying and navigating generic services
