About the role
Key responsibilities & impact- Lead Early Start Applicant Assessment
- Contact clients/families to make appointments
- Conduct interviews for home-based assessments
- Design and monitor Individual Family Service Plans (IFSPs)
- Work with families to assess needs and preferences
- Complete Early Start Assessment reports
- Coordinate assessment meetings and necessary documentation
- Empower applicants and families in resource coordination
- Participate in community meetings and other internal meetings
Requirements
What you’ll need- Master’s Degree in Social Work, Early Childhood Education, or related human services field preferred
- Bachelor’s degree in social work with 1 year of experience working with 0-36 month population required
- 2+ years of experience working with developmentally disabled population
- Interviewing techniques for person-centered thinking
- Knowledge of counseling principles and techniques
- Ability to maintain case records clearly
- Written and oral communication skills
- Basic familiarity with MS Office applications
