About the role
- Support complex CKD/ESKD patients and assist them in navigating the healthcare system
- Conduct medical record review and consultation to identify medical, social, emotional, and financial needs
- Work autonomously and collaborate with healthcare team to coordinate and facilitate quality, consistent, cost-effective care
- Work from home (telehealth) and travel within assigned geography for in-person visits to dialysis clinics and nephrology practices
- Coordinate care transitions, manage complex/at-risk patients, establish treatment plans with care team
- Identify and address social determinants of health including medical, emotional, education, community, and financial concerns
- Assess patients to identify unreported medical conditions or changes that may lead to adverse outcomes
- Build empathetic relationships with patients and families
- Monday-Friday schedule with ability to accommodate patient and nephrology partner availability
- Travel within assigned territory for in-person visits
Requirements
- Must live locally
- Current RN License required
- BSN preferred
- Minimum of 2 years RN experience required with healthcare performance coaching
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
- Current driver’s license and willingness to travel within an assigned territory
- Intermediate computer skills and proficiency in MS Word, Excel & Outlook required
- Home office with internet connectivity at a minimum of 1MB upload and 1MB download speed required
- Passion for delivering care to patients with complex, chronic illnesses
- Flexible and willing to pivot quickly on priorities
- Preferred: 2+ years of Case Management or Chronic Care Management
- Preferred: 1+ years of utilization management experience
- Preferred: Spanish-speaking in some markets
