Provide Physical Therapy services under the direction of a Physical Therapist according to the attending physician's Plan of Care
Work with clients and family in the home setting without direct supervision and adapt treatments to the unique home setting
Demonstrate competency in intervention, implementation of the treatment plan established by the PT, documentation, and communication with clients and PT
Identify and make appropriate referrals regarding medical, social and emotional factors influencing care
Communicate with the multidisciplinary team to form a collaborative client care plan and coordinate care with other team members
Assess effectiveness of treatment and notify PT if treatment needs modification; plan for discharge and participate in team reassessments and discharge planning
Teach client or family discharge therapy program and recommend/facilitate appropriate medical and/or rehabilitation follow-up
Document all treatment and coordination of care contacts in appropriate agency format and meet agency billing and reimbursement requirements
Independently manage caseload under supervision of a physical therapist, plan and schedule home visits, and organize weekly schedule to attend required meetings
Respond to urgent client needs by prioritizing and triaging treatments; communicate schedule and work plan to scheduling coordinator and supervisor
Assist in obtaining and fabricating adaptive equipment; adapt equipment and techniques for home use
Educate facility/agency staff, hospital staff and community resources regarding therapy services for home care/hospice clients
Comply with all applicable Federal, State, and local laws, regulations, HIPAA rules, and facility/agency policies and procedures
Requirements
Associate degree in Physical Therapy from an institution of higher learning accredited by the Commission on Accreditation in Physical Therapy Education
Current, unencumbered (State Employed) License as a Physical Therapy Assistant
Preferred - 1 year of Physical Therapy Assistant experience in a home care or rehabilitation setting
Preferred within 30 days of hire - National Provider BLS - American Heart Association (Vendor Managed)
Ability to understand, read, write, and speak English; Requires ability to communicate in English effectively both verbally and in writing
Demonstrated knowledge of relevant rehabilitation therapy techniques and procedures
Ability to add, subtract, multiply, and divide in all units of measure, compute rate, ratio, and percentage
Ability to write reports, business correspondence, and procedure manuals; Strong written and verbal communications
Ability to lift and move objects up to 50 pounds; regular standing, walking, and manual dexterity required
Ability to practice universal precautions, infection control, sanitation, and safety and disaster preparedness
Ability to prioritize work assignments and meet productivity and quality standards; meet agency time guidelines for documentation
