Completes comprehensive assessments of patients and families to determine hospice needs, including physical assessments and histories of current and previous illnesses.
Develops, initiates, and revises individualized Plans of Care in collaboration with the IDG, using health assessment data to determine nursing diagnoses and establish goals.
Provides professional nursing care by utilizing the nursing process, implementing palliative interventions, administering prescribed medications and treatments, and counseling patients and families on health care needs.
Provides health care instructions to patients and families as appropriate per assessment and plan.
Coordinates with the Patient Care Manager and IDG to ensure continuity of care, safe environments, and support for the emotional needs of patients and caregivers.
Completes, maintains, and submits accurate and timely clinical documentation, including patient condition, care provided, and pain/symptom management outcomes.
Communicates patient needs, condition changes, and physician orders to the IDG and supervisors; attends IDG meetings to review and update Plans of Care.
Fulfills accepted case assignments, participates in on‑call duties, and maintains productivity standards per organizational guidelines.
Supervises and delegates responsibilities to ancillary personnel as appropriate.
Supports the orientation, education, and development of team members, and provides input on CHHA performance and assignments.
Assumes responsibility for professional growth through continuing education, in‑service programs, and other activities to enhance job‑related skills and abilities.
Actively participates in performance improvement initiatives and adheres to the Corporate Integrity Plan.
Ensures financial prudence in care delivery by coordinating medications, supplies, equipment, and other necessary services defined in the Plan of Care.
Requirements
Education sufficient to meet certification, licensure, or registration requirements.
Preferred: 1 year of recent experience in hospice, palliative care, home health, acute care, and/or skilled nursing as a registered nurse.
National Provider BLS – American Heart Association (Vendor Managed) required within 30 days of hire.
Active state RN license (Vendor Managed) required within 30 days of hire.
Active Registered Nurse License (Vendor Managed) required upon hire.
Ability to understand, read, write, and speak English.
Visual acuity and hearing to perform required nursing skills.
Ability to lift and move objects up to 50 pounds.
Preferred working knowledge with computer applications, i.e. Microsoft Word/e-mail.
