Requirements
Bachelor's degree in Occupational Therapy OR Master's degree in Occupational Therapy
Current, unencumbered License as an Occupational Therapist in Washington state
National Provider BLS certification (to be obtained within 30 days of hire)
National Provider Identifier (NPI) number (to be obtained within 30 days of hire)
1 year of Occupational Therapy experience with adults in a home care setting preferred
