About the role
- Provide home care services and case management for homebound patients within South Chicagoland
- Assess, plan, implement, and evaluate nursing care in patients' homes
- Coordinate medical needs and utilize team resources (social worker, CNA, MD, etc.)
- Document and connect with the care team through point of care documentation
- Develop and maintain therapeutic relationships with patients and caregivers
- Set holistic patient and family goals and facilitate quality, cost-effective care
- Provide patient and staff education; utilize resources to meet learning needs
- Demonstrate nursing process as a problem-solving model and clinical mastery
- Assume leadership responsibilities at the unit level and participate in interdisciplinary collaboration
- Make visits to private residences and travel across the service area; respond to on-call and rotating schedules
Requirements
- Graduate from an accredited school of nursing
- One year of medical surgical experience in an acute or subacute care facility
- Current Illinois RN license
- Current CPR
- Earn 10.0 continuing education credits annually
- Ability to lift up to 35 pounds without assistance (patient handling equipment expected for lifts over 35 pounds)
- Ability to assist with ambulation including turning and transferring patients up to 200 pounds
- Ability to lift up to 100 pounds occasionally and/or in excess of 50 pounds frequently and/or in excess of 20 pounds constantly (physical requirements listed)
- Must be able to work one summer and one winter holiday
- Must be able to rotate on an on call schedule
- Ability to work weekends, holidays, and different shifts
- Ability to take on-call assignments
- Ability to travel to all areas within the Advocate service area in all extremes of weather on all types of roads
- Required to make visits to private residences which may not be accessible to people with disabilities
