• This is a full-time role for a Home Health and Hospice Sales Consultant located in Michigan. The primary responsibilities include identifying and developing relationships with referral sources, generating new business leads, promoting the services of AdvisaCare Home Health Care, and maintaining excellent communication with clients and healthcare professionals. Additional tasks involve account management, preparing and delivering presentations, meeting sales goals, and working collaboratively with the team to achieve organizational objectives.

• - Staying up to date with the latest research and best practices in ABA

Social Worker

• **🎯 Your Key Responsibilities**
• - Conduct comprehensive psychosocial assessments to identify patient and family needs, develop individualized care plans, provide interventions, and monitor progress toward patient goals.
• - Deliver skilled medical social work services in accordance with physician orders and the CMS Conditions of Participation (CoPs).
• - Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards.
• - Collaborate with patients, caregivers, physicians, and the interdisciplinary team to promote safe, effective care and successful discharge planning.
• - Provide counseling, emotional support, caregiver education, crisis intervention, and referrals to community resources, financial assistance, transportation, Durable Medical Equipment, long-term care, and other support services.
• - Advocate for patients, address psychosocial barriers to care, and communicate changes in patient status to the care team.

Home Health Medical Social Worker – MSW

Speech Language Pathologist

• Conduct comprehensive speech-language evaluations to assess communication, cognitive, and swallowing needs; develop individualized treatment plans, provide evidence-based interventions, and monitor patient progress.
• Deliver skilled speech therapy services in accordance with physician orders and the CMS Conditions of Participation (CoPs).
• Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards.
• Collaborate with patients, caregivers, physicians, and the interdisciplinary team to optimize communication, cognition, swallowing, safety, and functional independence.
• Educate patients and caregivers on communication strategies, swallowing precautions, cognitive techniques, adaptive tools, and individualized Home Exercise Programs (HEPs) to support functional recovery.
• Reassess patient progress, modify treatment plans as appropriate, and communicate changes to the care team.

Home Health Speech-Language Pathologist, SLP
