Change lives- one home at a time.**Join the AdvisaCare team!** We're hiring passionate **Home Health Occupational Therapists (OTs)** and **Occupational Therapy Assistants (OTAs)** in **Oakland, Macomb, Wayne and Washtenaw** counties to deliver personalized, one-on-one care that helps patients live safely and independently at home.   • 🎯 Your Key Responsibilities:
  • - Conduct comprehensive evaluations to assess functional needs, develop individualized treatment plans, deliver evidence-based interventions, and monitor progress to maximize patient independence, safety, and quality of life.
  • - Deliver evidence-based occupational therapy in accordance with physician orders and the CMS Conditions of Participation (CoPs).
  • - Ensure timely, accurate documentation and maintain compliance with CMS, payer, and agency standards.
  • - Collaborate with patients, caregivers, physicians, and the interdisciplinary team to maximize function and independence.
  • - Educate patients and caregivers on ADLs, home safety, adaptive equipment, fall prevention, and individualized Home Exercise Programs (HEPs) to promote safety, independence, and functional recovery..
  • - Monitor patient progress, update treatment plans as needed, and communicate changes to the care team.

Requirements

What you’ll need
  • 💼 What We're Looking For:
  • - Degree from an accredited Occupational Therapy program.
  • - Current Michigan Occupational Therapist license in good standing.
  • - Home health experience preferred (minimum one year).
  • - Strong clinical judgment, critical thinking, and problem-solving skills.
  • - Excellent communication, interpersonal, and patient education skills.
  • - Ability to work independently while collaborating effectively with an interdisciplinary team.
  • - Reliable transportation, a valid driver's license, and current auto insurance.
