About the role
Key responsibilities & impact- Restore movement and rebuild independence in patients
- Conduct comprehensive physical therapy evaluations to assess needs and develop treatment plans
- Deliver skilled physical therapy services per physician orders
- Maintain accurate documentation and ensure compliance with standards
- Collaborate with patients, caregivers, and the interdisciplinary team
- Educate patients and caregivers on therapeutic exercises and recovery techniques
- Reassess patient progress and modify treatment plans as needed
Requirements
What you’ll need- **💼 What We're Looking For:**
- - **Degree from an accredited Physical Therapy (PT) or Physical Therapist Assistant (PTA) program.**
- - **Current Michigan Physical Therapist/Physical Therapist Assistant license in good standing.**
- - **Home health experience preferred (minimum one year).**
- - **Strong clinical judgment, critical thinking, and problem-solving skills.**
- - **Excellent communication, interpersonal, and patient education skills.**
- - **Ability to work independently while collaborating effectively with an interdisciplinary team.**
- - **Reliable transportation, a valid driver's license, and current auto insurance.**
