About the role

Key responsibilities & impact

Restore movement and rebuild independence in patients
Conduct comprehensive physical therapy evaluations to assess needs and develop treatment plans
Deliver skilled physical therapy services per physician orders
Maintain accurate documentation and ensure compliance with standards

What you’ll need

**💼 What We're Looking For:**
- **Degree from an accredited Physical Therapy (PT) or Physical Therapist Assistant (PTA) program.**
- **Current Michigan Physical Therapist/Physical Therapist Assistant license in good standing.**
- **Home health experience preferred (minimum one year).**
- **Strong clinical judgment, critical thinking, and problem-solving skills.**
- **Excellent communication, interpersonal, and patient education skills.**
- **Ability to work independently while collaborating effectively with an interdisciplinary team.**
- **Reliable transportation, a valid driver's license, and current auto insurance.**
